Dear Mr. Sexton:
On November 4, 2014, Jenny Graznak of this office performed a Compliance Evaluation Inspection
on the General Permit to discharge domestic wastewater from the single family residence located at
129 Forestdale Drive in Reidsville, NC. According to county records, you are the current owner of
this property, and you were present for the inspection.
Ms. Graznak observed two chlorinator tubes as well as the effluent pipe. You stated that you
regularly add chlorine tablets to the system. The Permit requires that the chlorinator be adequately
supplied with chlorine tablets for proper operation. The Permit also requires that the septic tank
contents be pumped out every five years to prevent solids from clogging the sandfilter system. Ms.
Graznak located the effluent pipe at the end of Forestdale Drive. No discharge was observed. The
inspector detected no obvious indicators of problems with the system. You stated that the septic tank
was pumped out in 2013.

During the inspection, you stated that you may be selling this residence in the coming year. Please
pass along the permit information to the next owners. In addition, the new owners will need to
complete a Permit Name/Ownership Change Request, which can be found on our website:
