February 13, 2007. Larry Frost and Keith Haynes of the Asheville Regional Office conducted the
Compliance Evaluation Inspection. The treatment facility was found to be in violation of Permit
NCG550040 for the following:

Permit Permit has been expired since 1997
Please refer to the enclosed Inspection Report for any additional observation and comments.
