I have been out of the office for several weeks and am following up with you about the 401 Certificate application for
the Ward Mill dam project. Attached is a scan of the signed page 5 which was inadvertently submitted without my
signature. I have also prepared an Agent Authorization form and sent it to the Wards to be executed. I asked them to
return it to me when they have signed it. When I get that, I will forward it to you. I appreciate your assistance.

Note: Wetlands or waters of the U.S. may not be impacted prior to: (1) the issuance of a Section 404 or
Section 1O permit from the U.S. Army Corps nfEngineers; (2) The issuance or waiver ofa 40I Water
Quality Certification from the NC Division of Water Resources, and (3) In the 2D Coastal Counties, aletter
from the NC Division of Coastal Management stating that the proposed activity is consistent with the NC
Coastal Area Management Act.
This project involved the continued operation mf the Ward Mill Dam Hydroelectric facility uoit has been
operated for over 25 yuaxm' There is no proposed construction, fill, or other disturbance of the stream or
wetlands.
14. Certification Fee
If the impact is less than one (1) acre of wetland or water and less than 150 linear feet of stream, enclose a
check for $240 made to NCDENR.
If the impact exceeds either or both of the levels above, enclose a check for $570 made toNCDENR.
15. Public Notice
Public Notice is required for all FER[projects. Please note that the applicant is required to reimburse the
Division of Water Resources for the costs associated with the Public Notice pursuant to 15A NCAC 02H
