Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
2005. This final permit includes no major changes from the draft permit sent to you on February 2,
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request Within thirty (30) days
following receipt of this letter. Thu request must be in the form of a written petition, conforming to Chapter
150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail
Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final
and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required bythe Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local

in accordance with effluent limitations, monitoring requirements, and other
conditions set forth in Parts I, II, III and IV hereof.
This permit shall become effective May 1, 2005.
This permit and authorization to discharge shall expire at midnight on April 30, 2010.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or
discharge are hereby revoked. As of this permit issuance, any previously issued
permit bearing this number is no longer effective. Therefore, the exclusive authority
to operate and discharge from this facility arises under the permit conditions,
requirements, terms, and provisions included herein.
Catawba County Schools is hereby authorized to:
1. Continue to operate an existing 0.0065 MGD wastewater treatment system with
the following components:
♦ Two septic tanks in series
♦ 1150-ft2 dosing tank with dual siphons
♦ Dual 1152-ft2 sand filter beds in parallel with liners, distribution piping &
underdrain
♦ Recirculating pump station with dual recirculation & dual discharge pumps
♦ Dual -module UV disinfection unit
This system is located at the Mill Creek Middle School WWTP (1041 Shiloh Road)
in Catawba County.
2. Discharge from said treatment works at the location specified on the attached
map into Balls Creek, classified WS-IV waters in the Catawba River Basin.

5. Directions to site: From the intersection of Highways 16 and 150, travel north on Highway 16
approximately 4 miles to Buffalo Shoals Road. Turn right on Buffalo Shoals Road and travel
approximately 3 miles to Shiloh Road which forks to the left. Travel approximately 150 yards
to school located on the left hand side of the road. The first access road to the left after the
school will lead to the WWTP.

The Permittee, Catawba County Schools, requests renewal of its NPDES permit to discharge
treated domestic wastewater from the wastewater treatment system serving Mill Creek Middle
School. A review of the past year's self -monitoring data, from 12/2003 through 11 /2004, revealed no
permit violations.
Pending review and approval by P&E, it is recommended that the permit be renewed as
requested.

Ref. Narrative/Sludge Use
Sludge is handled by a septic tank pumper and is disposed of in a municipal plant.
At this time Sanitary Septic Tank Inc., pumps the tanks if needed once a year and
disposes of the sludge in one of the Hickory Wastewater Treatment Plants.
