North Carolina, having been authorized to observe (periodically, weekly, full time) the construction

Permittee hereby state that, to the best of my abilities, due care and diligence was used in the
observation of the following installations:
• Two (2) septic tanks, nominal sizes: 1600 gallons and 3200 gallons.
• An 11.5 ft x 10 ft dosing tank with dual siphons.
• Two (2) 1152 sq. ft. each sand filter beds with liners, distribution piping and underdrains.
• A recirculating pump station with dual recirculation and dual discharge pumps.
• A dual module UV disinfection unit.
• On -site potable water with back -flow prevention.
• Electrical controls, manholes, cleanouts, piping, valves and appurtenances.
As required in the this ATC, the following requirements have been met:
1. Storing spare UV racks and bulbs on site for use in emergency or failure.
2. Installation of a grease trap if the school will have a cafeteria (under this contract or another
contract).
I certify that the construction of the above referenced project was observed to be built within
substantial compliance and intent of the approved plans and specifications.

The Division received a letter of request for an Authorization to Construct October 6, 1998 and final
plans and specifications for the subject project have been reviewed and found to be satisfactory.
Authorization is hereby granted for the construction of a new 0.0065 MGD wastewater treatment
facility at the East Catawba Middle School in Catawba County with discharge into the Balls Creek in
the Catawba River Basin.
Specifically, authorization is granted for the construction/installation of -
Two (2) septic tanks, nominal sizes: 1600 gallons and 3200 gallons.
An 11.5 ft x 10 ft dosing tank with dual siphons. .
Two (2) 1152 sq. ft. each sand filter beds with liners, distribution piping and underdrains.
A recirculating pump station with dual recirculation and dual discharge pumps.
A dual module UV disinfection unit.
On -site potable water with back -flow prevention.
Electrical controls, manholes, cleanouts, piping, valves and appurtenances.
This ATC is issued contingent upon the following requirements being met:
1. Cancellation of classes during power outages.
2. Storing spare UV racks and bulbs on site for use in emergency or failure.
3. Installation of a grease trap if the school will have a cafeteria (under this contract or another
contract).
This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES Permit
No. NCO086304 issued September 25, 1998, and shall be subject to revocation unless the wastewater
treatment facilities are constructed in accordance with the conditions and limitations specified in
Permit No. NC0086304.
The sludge generated from these treatment facilities must be disposed of in accordance with G.S.
143-215.1 and in a manner approved by the North Carolina Division of Water Quality.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required

by this Division, such as the construction of additional or replacement wastewater treatment or
disposal facilities.
The Mooresville Regional Office, telephone number (704) 663-1699, shall be notified at least forty-
eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can be
made. Such notification to the regional supervisor shall be made during the normal office hours from
8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.
Upon completion of construction and prior to operation of this permitted facility, a certification must
be received from a professional engineer certifying that the permitted facility has been installed in
accordance with the NPDES Permit, this Authorization to Construct and the approved plans and
specifications. Mail the Certification to the NPDES Unit, P.O. Box 29535, Raleigh, NC 27626-0535.
Upon classification of the facility by the Certification Commission, the Permittee shall employ a
certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater
treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or
greater than the classification assigned to the wastewater treatment facilities by the Certification
Commission. The Permittee must also employ a certified back-up operator of the appropriate type
and grade to comply with the conditions of Title 15A, Chapter 8A, .0202. The ORC of the facility
must visit each Class I facility at least weekly and each Class II, III, and IV facility at least daily,
excluding weekends and holidays, and must properly manage and document daily operation and
maintenance of the facility and must comply with all other conditions of Title 15A, Chapter 8A,
.0202. Once the facility is classified, the Permittee must submit a letter to the Certification
Commission which designates the operator in responsible charge within thirty days after the
wastewater treatment facilities are 50% complete.
The Operational Agreement between the Permittee and the Environmental Management Commission
is incorporated herein by reference and is a condition of this Permit. Noncompliance with the terms
of the Operational Agreement shall subject the Permittee to all sanctions provided by G. S. 143-215.6
for violation of or failure to act in accordance with the terms and conditions of this Permit.
Failure to abide by the requirements contained in this Authorization to Construct may subject the
Permittee to an enforcement action by the Division of Water Quality in accordance with North
Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from complying with
any and all statutes, rules, regulations, or ordinances which may be imposed by other government
agencies (local, state, and federal) which have jurisdiction.
One (1) set of approved plans and specifications is being forwarded to you. The Pennittee shall
maintain a copy of the approved plans and specifications on file for the life of the facility. If you

Dear Mr. Gilbert:
As discussed during the preconstruction conference, we have obtained approval from the
N.C. Department of Envirionment & Natural Resoures (NCDENR) to modify the
specification for sand filter media. The media currently specified is not widely available.
Acceptable media for the sand filter include fine filter sand with an effective size between
0.35-0.60 mm, a uniformity coefficient less than 3.0, and dust content less than 0.5
percent. This sand is available from several suppliers throughout North Carolina. A
partial list of suppliers is attached. Please call if you have any comments or questions.

Enclosed are three (3) copies each of the revised Drawings and Specifications for the above
referenced project. We offer the following in response to your review comments received by
phone on December 17, 1998:
V 1. Will grease trap be installed under separate contract?
The grease trap will be installed under separate contract. It was designed by McKnight,
Smith & Ward. On the previous submittal we provided a detail and a copy of the submittal
drawing for the grease interceptor.
✓ 2. Need copy of design calculations. Previous copy destroyed in move.
Enclosed are three copies of the project design calculations. Professional Engineer.
3. Need copy of sand filter specification.
Three copies of the project specifications are enclosed. Specifications for the sand filter bed
can be found in Section 13360.2.05.
4. Per Regional Office comment: Flow based on 10 gpd per student should be ok for this
project. Typically like to see flow based on 12 or 15 gpd.
The NPDES Permitted flow for this project is 10 gpd per student.

Is potable water supplied by well?
The school is served by a municipal water system. Wells are not utilized at the school.
6. Provide detail of clean -out.
A clean -out detail has been added to Sheet 4 of 7 of the revised drawings.

A detail of the rip -rap apron has been added to sheet 4 of 7 of the revised drawings.
8. Provide detail of sand filter liner. Specifically, how does it connect to the wall?
The 30 mil polyethelene liner will extend up the walls of the sand filter. The detail on sheet
e Tof 7 has been revised to reflect the liner.
9. Recommend sampling point be made easily accessible — not too heavy of a lid, not too
deep.
A sample station has been added at station 1+30 and is shown on sheet 4 of 7. This pre -cast
concrete box will be 5.3' feet deep. A 2'x3' lockable aluminum access door has been
specified to facilitate ease of use by the operator.

Though not required by the Division of Water Quality, we have chosen to include the
continuous recording flow meter at this time.

Enclosed are three (3) copies each of the revised Drawings and Specifications for the
above referenced project. In response to preliminary review comments, the following
revisions have been incorporated into the drawings and specifications:
1. The specifications have been sealed by a NC Professional Engineer.
L- 2. System redundancy has been enhanced by using dual alternating pumps for
both recirculation and discharge lines.
,--� A eg nerator receptacle is included in the project to provide astand-by power
source for operating the pump station and disinfection system in the event of a
power outage. A long-term power outage would result in the school being
closed.
✓ 4. The proposed treatment system is not located in the 100 year flood plain
according to FIRM Map 370050 0165B. Please refer to the attached letter
from the Catawba County Zoning Administrator and copies of the Catawba
County North Carolina (unincorporated areas) Map Index.
5. The proposed treatment system is located 100 feet from the nearest spring. It
is further than 25 feet from the drainage ditch located downslope. It is further
than 100 feet from the pond fed by the spring. We believe that all buffer
requirements listed in 2H.0219 have been met.
6. A 110 volt power source is provided on the pump panel as indicated in the
specifications. A potable water source with back flow prevention is shown on
the plans.
Additionally, other revisions have been made to the plans based on our several phone
conversations about this and other projects. These revisions are indicated below:
7. Manhole #5A has been added between manholes #5 and #6.
8. An effluent filter has been added to the second septic tank in series.
9. Clean -outs have been added to the 6-inch distribution piping.

10. Ball valves have been added to each lateral line at the sand filter bed.
11. A continuous flow meter with data recorder has been added to the discharge
line.
We are enclosing a detail of the grease interceptor that was photocopied from the
architectural drawings. Design of this component is the responsibility of the
plumbing/mechanical consultant to the architect preparing the school addition/renovation
plans. Also enclosed is a copy of the submittal drawing for the grease interceptor.
Questions should be addressed to me at this office. Due to the compressed time schedule
necessary to complete construction before school starts next fall, we appreciate your
assistance in expediting this review. Thank you for your cooperation.
