Dear Ms. Knighton:
The Division received your application for a wastewater discharge permit on July 18, 1997.
Division personnel have reviewed and approved your application. Accordingly, we are forwarding herewith
the attached NPDES discharge permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215 .1 and the Memorandum of Agreement between North Carolina and the
U.S. Environmental Protection Agency dated December 6, 1983, and as subsequently amended.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings,
Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this
decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. Complete and
submit the enclosed Permit Name / Ownership Change Form if a change in ownership or control of this
discharge occurs. The Division may require modification or revocation and reissuance of the permit.
This permit does not affect the legal requirements to obtain other permits which may be required
by the Division of Wgter Quality or permits required by the Division of Land Resources, the Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.

Parts I, II, III, and IV hereof.
This permit shall become effective May 1, 1998.
This permit and authorization to discharge shall expire at midnight on November 30, 2002.

and 002. The facility monitors for flow, pH, TRC and TSS (2/month) and TP, TN,
and Nitrate/Nitrite (quarterly).
11/24 Drafted pernut: Removed the pH monitoring requirement.
Added monitoring for Iron, chloride and TRC.
Added monitoring frequencies for all parameters.
Facility requested clarification in the per-mit of the two plants, 10/14/91. Incorporated this.
12/1 Wrote draft cover letter with changes and informing facility that TP, TN, and Nitrate/Nitrite
monitoring is not required.

1/8/98 Called Jill Hickey. AG, per Dave's request about legality of one permit for two
outfalls at two plants.
1/23 Called Jill Hickey again. She says there probably should be 2 permits for 2
facilities, but it doesn't specifically state this in the regs.
Dave wants me to call RO and scc if they have any problems with scparating this
into 2 permits. Left a message with Mark Starr. He called back - discharge is
within 1000'; he states that in these, cases, RO leaves it as one permit. He will call
back with pennit cumber if he thinks of another permit with the same condition.
1/26 Left a message with Roger Thorpe about whether to issue 2 permits or not.
1/29 Roger Thorpe is OK with issuing 2 permits.

Connection to Regional Sewer System: Possible, but salts from water softener would most likely
disrupt biology of waste water treatment plant and shorten the life of the sprayfield. Therefore not
a recommended option.

PART IV - EVALUATION AND RECOMMENDATIONS
The results of the inspection indicate that the plant was in compliance with the limitations and conditions of
the permit. In general, the plants are well maintained. The effluent from both plants appeared clear and free
of visible solids. The biota of Filbert Creek did not seem to be affected by the discharges. A cursory review
of data generation and tracking, sampling practices, and self -monitoring data revealed no discrepancies. It is
recommended that the permit be renewed for a period of five years.
