Robert Tankard and Sarah Toppen with DWR inspected the wwtp with Michele Pharr, contract operator for Environlink. The mechanical bar screen was still in need of repair. Chicken wire was being used to screen the influent prior to entry into the eq basin. The eq pumps had been repaired and the alarms worked. The operator measured the sludge blanket in the clarifiers. There was approx. 11 and 10 feet of sludge in each of the two clarifiers. The disinfection system was being used. Could not see if there were tablets in the tubes or not due to safety reasons. The clear well had solids in the basin. The operator was asked to backwash the filters while the inspection was ongoing. She could not get the filters to backwash. She had to call Mr. Chuck Bryant with Envirolink for assistance. The backwash occurred. Not sure how often the filters are backwashed. The filters never cleared of solids. Mr. Bryant with Envirolink at the previous inspection stateed that the backwash was blowing media into the mud well. An evaluation of the filter media must be conducted. The permit expired and has not been reapplied for! No lab sheets were available. Operator's log only had the week of inspection. The back up operator had visited the site on that Tuesday and then on that Thursday (inspection day). Therefore the operator would not meet the five days requirement for visitation for the week. The wwtp generator is non operational. The wwtp is not secure from the public. The neighbor was using the road and the adjacent property as a park and ride area. The wwtp is not fenced. The 64,000 gpd wwtp was not operational and trees were growing out of the tanks. The high rate infiltration field has not been corrected. The wetted acreage does not meet the permit requirements. Also, there was vegetation growing in the fields. Also, solids and ponding were observed. The irrigation nozzles had been replaced but the water was still ponding approximately 5 feet around the stand pipe. The facility is not meeting reclaim standards. The permittee is not meeting condition III. 2. The operator shall visit the facilities in accordance with 15A NCAC 08G .0200 and shall comply with all other condtiions specified in these rules. * The opeator shall visit the system as often as is necessary to ensure the proper operation of the system but in no case less than five days per week. *operate and maintain the system and attempt to ensure the compliance of the system with any permits issued for the system as well as any other applicable local, State, and federal environmental permitting and regulatory requirements; *certify by signature the validity of all monitoring and reporting information performed on the system as prescribed in any permit issued for the system and provide the owner a copy of monitoring and reporting forms; * document the operation, maintenance, and all visitation of the system in a log that shall be maintained at the system; * notify the owner of the system within 24 hours and in writing within five days of first knowledge, of any: *notify the owner in writing of the need for any system repairs and modifications that may be necessary to ensure the compliance of the system with all local, State, and federal environmental permitting and regulatory requirements; *to provide access to the facility to regulatory agencies Page 2 of 8 Permit: WQ0015052 Owner - Facility: Enviro-Tech of North Carolina Inc Inspection Date: 09/28/2023 Inspection Type: Compliance Sampling Reason for Visit: Routine *The back up operator shall not exceed 40 percent of the system visitations required per calendar year under Rule .0204(2) of this Section; The permittee is not meeting permit condition IV. 9. Records shall be maintained of all residuals removed from the facility. Records shall include the name of the hauler, permit authorizing the disposal or a letter from a municipality agreeing to accept the residuals, date the residuals were hauled, and volume of residuals removed. The permittee is not meeting permit conditin IV. 10. A maintenance log shall be maintained at the facility. The maintenance log that was presented was only for the month of September 2023. MULITPLE REPORTING AND MONITORING VIOLATIONS FOR THE SAMPLING AND REPORTING HAVE OCCURRED OVER THE LAST YEAR. NOTICES WILL BE SENT UNDER SEPARATE LETTER. The facility was considered non -compliant.
