The North Carolina Division of Water Resources conducted an inspection of the 4 Hidden Creek Drive (20 Hidden
Creek Drive) on September 21, 2023. This inspection was conducted to verify that the facility is operating in
compliance with the conditions and limitations specified in NPDES WW Permit No. NCG550466. A summary of the
findings and comments noted during the inspection are provided in the enclosed copy of the inspection report.

The Compliance Evaluation inspection was conducted by Division of Water Resources staff from the Asheville
Regional Office. The following deficiency(s) was noted during the inspection:

Remedial actions should have already been taken to correct this problem and prevent further occurrences in the
future. The Division of Water Resources may pursue enforcement action for this and any additional violations of
State law. To prevent further action, carefully review these deficiencies and address the causes of
non-compliance to prevent the recurrence of similar situations.

Lauren Armeni, Mara Chamlee, and Rachel Rose, with the Asheville Regional Office, conducted a
Compliance Evaluation Inspection of the single-family wastewater disposal system that services 4
Hidden Creek Drive. This inspection was conducted to determine if the system is being operated and
maintained in compliance with General NPDES Permit No. NCG550000. Mr. Robert Bongers was
present during the inspection.
Permit: There are past due invoices that need to be paid in order to renew this Certificate of
Coverage under General Permit NCG550000. The past due invoices will be attached to this report.
Note that on the Buncombe County GIS this property is listed as 20 Hidden Creek Drive.
Septic Tank: At the time of the inspection, tank maintenance or pumping records were not available.
Ms. Armeni contacted Able Septic to determine when the tank was last pumped here, and she was
told the last time was in May 2017. The solids level in the septic tank needs to be checked to
determine if pumping is necessary.
Effluent Pipe: At the time of the inspection, the effluent pipe could not be located. The last report in
2018 indicated that the effluent pipe had been located via dye testing; however, the pipe was not
marked and could not be found. The pipe needs to be located again and marked in order to inspect
it.
The following deficiencies were noted during the inspection:
1. Septic Tank: At the time of the inspection, no maintenance or pumping records were available.
2. Effluent Pipe: The effluent pipe could not be located during the inspection.

is expired because there are multiple past due annual permit fees that need to be paid.

instructions on the invoice.

level is high enough to turn on the effluent pump. The effluent pipe exiting the contact
chamber is not adequately sloped to allow water to gravity feed to the receiving stream.

At the time of the inspection, the effluent pipe could not be located; however, the 2018
report indicated that the effluent pipe is located across Hidden Creek Drive by the
culvert. Mr. Bongers had previously confirmed the location using dye. It is
recommended to confirm the location again and mark it so it can be inspected in the
future. See summary for details.

checked/added. If at any time the water level reaches the discharge pipe, sampling is
required.
