A. Project Description, Location, and Purpose:
The North Carolina Department of Transportation (NCDOT) proposes to replace Bridge No.
254 on Wood Bridge Road (S.R. 1512) over Third Creek in Iredell County (see Figure 1).
The existing timber -deck bridge will be replaced with a new concrete box beam bridge that
will accommodate two 11-foot lanes. The proposed bridge will be placed on the same
alignment and will be widened by approximately 1-foot on both sides. The approaches will
be widened to include two 11-foot lanes plus 2-foot paved shoulders. Class II Riprap is
proposed along the channel banks.
This project is funded for $1,254,000, with $114,000 allocated for preliminary engineering
and $1,140,000 allocated for construction. Right-of-way is anticipated in Fiscal Year (FY)
2023 and construction is anticipated in FY 2025.
The purpose of this project is to replace the structurally deficient and functionally obsolete
bridge. Bridge No. 254 was constructed in 1962 and is approximately 60 years old. The
bridge is considered structurally deficient due to a structural evaluation of 4 out of 9 and a
substructure rating of 5 out of 9. It is currently posted at 9 tons Single Vehicle Truck (SV)
and 12 tons Tractor Trailer Semi Truck (TTST).

D. Changes in Proposed Action & Environmental Consequences:
Since the Minimum Criteria Determination Checklist (MCDC) was completed in May 2019,
no design changes have been made.
Anticipated Permits: Due to the anticipated impacts to jurisdictional streams, a US Army
Corps of Engineers (USACE) Regional General Permit 50 is anticipated for a bridge to
bridge replacement.

Estimated Cost: The most up to date estimated construction cost is $1,140,000.
Floodplain Management: Third Creek is a limited detailed FEMA stream included in Flood
Insurance Study (FIS) Number 37097CV000 for Iredell County dates effective March 18,
2008 and revised June 16, 2009. A Memorandum of Agreement was submitted and
approved in connection with the replacement of the SR 1512 (Wood Bridge Road) bridge
over Third Creek in Iredell County. Based on the results of this analysis, the proposed
project was shown to meet the requirements of a MOA Type 1.
Cultural Resources: The NCDOT Cultural Resources Group has indicated that no historic
architecture or archaeology survey is required. Since 5 years have passed, a 2022 request
was submitted and confirmed that this stands true.
Federally Protected Species: At the time of approval of the MCDC, the USFWS listed the
bog turtle, the dwarf -flowered heartleaf, and the Northern Long-eared Bat (NLEB). Species
listed as threatened due to similarity of appearance do not require Section 7 consultation
with the USFWS; therefore, a biological conclusion is not required for bog turtle. Since
approval of the MCDC, Schweinitz's sunflower has been added, and NLEB has been
removed. Tricolored bat has been added as proposed endangered.
Suitable habitat for dwarf -flowered heartleaf is present in the study area, however no
individuals were observed during pedestrian surveys conducted on May 2, 2016 and April
28, 2022. Suitable habitat for Schweinitz's sunflower is present in the study area, however
no individuals were observed during pedestrian surveys conducted on August 26, 2022.
Due to the lack of known occurrences, and lack of surveys that show occurrence within the
project study area, NCDOT recommended this project will have No Effect on dwarf -
flowered heartleaf and Schweinitz's sunflower.
A habitat assessment and structure survey were completed for the tricolored bat. Due to
the lack of utilization evidence within the structure and lack of known occurrences, the
biological conclusion for this species is recommended to be May Affect, but is Not Likely to
Adversely Affect.
E. Conclusion:
The above SEPA documentation has been reevaluated as required by NC General Statute
Chapter 113A Article 1. It has been determined that the current proposed action is
essentially the same as the original proposed action. Proposed changes, if any, are noted
in Section C. It has been determined that anticipated social, economic, and environmental
impacts were accurately described in the above referenced document(s) unless noted
otherwise herein. Therefore, the original Administration Action remains valid.
F. Coordination
The Division12 personnel have discussed the current project parameters with qualified
NCDOT representatives and FHWA (where applicable). The Division 12 Bridge Program
Engineer, Joshua White, PE, PLS, hereby verifies the involvement of the following staff and
the incorporation of their technical input:
