Dear Mr. Stotesberry:
The North Carolina Division of Water Resources conducted an inspection of the Swan Quarter Sanitary
District WWTF on 9/26/2023. This inspection was conducted to verify that the facility is operating in
compliance with the conditions and limitations spedfied in Non -discharge Permit No. WQ0019665. The
findings and comments noted during this inspection are provided in the enclosed copy of the inspection
report entitled "Compliance Inspection Report".
Please respond in writing to this office within 30 days of your receipt of this letter regarding your plans
or measures to be taken to address the following issues:
- The Influent/EfFluent flow meter equipment did not have a calibration sticker or calibration records on site. This
causes the fadlity to be in violation of Condition III.7 of Permit No. WQG019665 which states Irrigation
equipment shall be tested and calibrated once per permit cycle. [ 15A NCAC 02T. 0507(d).. Such records are
being requested as a follow-up to inspection.
- Record keeping was not evaluated during inspection since NDMR and lab sheets are not kept on site. Such
records are being requested as a follow-up to inspection. Please submit the following records for review:
o Operator visitation logs for 2022 and 2023
o Operator Operation and Maintenance logs for 2022 and 2023
o Lab data sheets for NDMR reporting forms
o Chain of Custody forms between SQSD and contracted lab for sample submissions
o Feld bench sheets for reported field parameters (Total Residual Chlorine,& pH)
o Annual Flow meter calibration documents for 2021, 202-2 and 2023.

- The liner In both lagoons is deteriorating and has multiple gas bubbles throughout. The liner is -pulling away -in
several places around both lagoons causing structural damage to the integrity of the berm wall. The condition
_issues with the -lagoon liner cause -the -facility to-be-in-violation--of-Condidorr1Ik-of-Rermit-No_WQO
which requires the facility to be properly operated as a non -discharge system preventing the -discharge -of -any
wastewater to ground or surface waters. f

Inspection Summary:
On September 26th, 2023, Victoria Herdt and Scott Vinson (WaRO) met with Mr. Joseph Sadler (ORC) and Jeff Stotesber y
for a routine compliance inspection. While onsite we also discussed reporting issues. The issues, namely lack of reporting,
should be easily remedied with proper attentlon from the Sanitary District and ORC. The spray fields appeared to be
adequate and well maintained. However, the condition of the lagoons continues to deteriorate. The bubbles do not appear to
have gotten any worse, but the liner is deteriorating and pulling away from the top of the berm in places. Notice of Violation
will be issued for lack of maintenanceflagoon Issues.
In regards to Permit #WQ0019665, Swan Quarter Sanitary District WWTF was not in compliance on the day of inspection.
