Quality REQUEST FORM
If a facility is required to sample multiple discharge locations with very similar storm water discharges, the
permittee may petition the Director for Representative Outfall Status(ROS). DEQ may grant Representative
Outfall Status if storm water discharges from a single outfall are representative of discharges from multiple
outfalls. Approved ROS will reduce the number of outfalls where analytical sampling requirements apply.
If Representative Outfall Status is granted,ALL outfalls are still subject to the qualitative monitoring
requirements of the facility's permit—unless otherwise allowed by the permit(such as NCG020000)and DEQ
approval. The approval letter from DEQ must be kept on site with the facility's Stormwater Pollution
Prevention Plan. The facility must notify DEQ in writing if any changes affect representative status.

North Carolina General Statute 143-215.6 B(i) provides that:
Any person who knowingly makes any false statement,representation,or certification in any application,record,
report,plan,or other document filed or required to be maintained under this Article or a rule implementing this
Article;or who knowingly makes a false statement of a material fact in a rulemaking proceeding or contested case
under this Article;or who falsifies,tampers with,or knowingly renders inaccurate any recording or monitoring device
or method required to be operated or maintained under this Article or rules of the [Environmental Management]
Commission implementing this Article shall be guilty of a Class 2 misdemeanor which may include a fine not to exceed
ten thousand dollars($10,000).
I hereby request Representative Outfall Status for my NPDES Permit. I understand that ALL outfalls are still
subject to the qualitative monitoring requirements of the permit, unless otherwise allowed by the permit
and regional office approval. I must notify DEQ in writing if any changes to the facility or its operations
take place after ROS is granted that may affect this status. If ROS no longer applies, I understand I must
resume monitoring of all outfalls as specified in my NPDES permit.

Please note: This application for Representative Outfall Status is subject to
approval by the NCDEQ Regional Office. The Regional Office may inspect your
facility for compliance with the conditions of the permit prior to that approval.
Final Checklist for ROS Request
This application should include the following items:

❑ Letter or narrative elaborating on the reasons why specified outfalls should be granted representative
status, unless all information can be included in Question 4.
❑ Two (2) copies of a site map of the facility with the location of all outfalls clearly marked, including the
drainage areas, industrial activities, and raw materials/finished products within each drainage area.
❑ Summary of results from monitoring conducted at the outfalls listed in Question 3.
❑ Any other supporting documentation.

The topography of the terrain within the Airport is generally flat. The Airport is located in the inner coastal plain of Eastern
North Carolina. Predominately located in a rural farming and agribusiness area in proximity to Buckskin Swamp. The
Airport is located within the Cape Fear River Basin, with Grove Creek approximately 0.5-mile southwest of airport
property, Beaverdam Branch approximately 0.5-mile north of Runway 5/23, and Buckskin Swamp flowing east and
crossing airport property. The airport is not located within a water supply watershed, and there are no restrictions on
watershed development activities in the project area.
Stormwater runoff from the western portion of the Airport; which includes the airport terminal, terminal apron, general
aviation hangars and associated taxiways; generally runs to the east within Buckskin Swamp under Runway 05-23.
Stormwater runoff from the east portion of the Airport; which includes Runway 5-23; flows east into Buckskin Swamp
Creek, which then flows through Grove Creek to the Northeast Cape Fear River. Buckskin Swamp begins west of Airport
property and then flows eastward before discharging into Grove Creek.
Stormwater runoff from the south portion of the Airport; which includes Runway 5-23 and Taxiway A flows south into a
riverine which empties into Grover Creek, which drains into Northeast Cape Fear River.
There are six stormwater outfalls from the Airport Property, listed below:

These six outfall locations are shown on the attached map, which was included in the 2011 SWPPP for the Airport.
The only new development on the property since the 2011 SWPPP occurred in the same area that all industrial activities
occurred in the past- the west side of the airport surrounding the airport terminal building. An additional map is provided to
show these new buildings and any hazardous materials used or stored at the new locations (for the updated SWPPP,
currently in progress, a new updated site map is being produced).
All hazardous pollutant substances that drain from the airport would be traced back to stormwater runoff originating in this
area of the property, via the same conveyances shown in the 2011 SWPPP site map, specifically, RC Pipe and grass-lined
swales. Contours for the new development did not change the direction of flow nor the specific outfall impacted by runoff.
The Airport respectfully requests assignment of Outfall #5 as the representative outfall for Outfalls 1, 2, 3, 4, and 6.
Stormwater runoff from much of the southern portion of the Airport flows south to Outfall #5. This area includes portions of
the runway and taxiway, the terminal buildings, parking lot, fuel farm, NC Forest Service hangars, and portions of the Jet
Techs maintenance hangar and the two corporate hangars. Outfall #5 is located at a 60-inch concrete headwall along the
south end of the runway. Outfall #5 ultimately discharges to Grove Creek. Monitoring is conducted at the headwall.
