FACT SHEET FOR EXPEDITED PERMIT RENEWALS
This form must be completed by Permit Writers for all expedited permits which do not require full Fact
Sheets. Expedited permits are generally simple 100% domestics (e.g., schools, mobile home parks, etc.)
that can be administratively renewed with minor changes but can include facilities with more complex
issues (Special Conditions, 303(d) listed water, toxicity testing, instream monitoring, compliance
concerns).

After our discussion with the Regional Office, NPDES Permittin a r d that fecal coliform limitations shall be put in the
Town Plymouth's j DES permit renewal L The limits on the effluent will be a geometric mean not to exceed 200 / 100 ml
.--as a monthly average and 400/100 ml as a weekly average using daily samples. The narrative special condition will be
removed. NPDES would prefer to not make this revision until we do the renewal in 2022 but if it is necessary that the
revision be done before that, so the Town can get the funding for the disinfection system, pl
ease let John Hennessy

The Town would either have to request the modification or NPDES would have to write an administrative letter to the
Town giving them a 60 day notification that DWR was going to modify the permit.
