Dear Mr. Hatchett:
The Division of Water Quality (the Division or DWQ) hereby issues the attached NPDES permit for
the subject facility. We issue this permit pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S.
Environmental Protection Agency dated October 15, 2007, or as subsequently amended.
The Division understands that you have made no significant alterations to your potable -water waste -
treatment plant (WTP) facilities during the previous permit cycle except to add dechlorination (ATC
May 4, 2009). For renewal we updated your permit [see permit Section A. (1.)] by applying the latest
statewide permitting strategy for WTPs specific to green -sand filter treatment technology (adopted
September 2009). This strategy includes a revised list of monitoring requirements, permit limits, and
effluent parameters of concern.
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable, you have the right to an adjudicatory hearing, upon written request submitted within
thirty (30) days after receiving this letter. Your request must take the form of a written petition
conforming to Chapter 150B of the North Carolina General Statutes, and you must file it with the
office of Administrative Hearings, 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
Unless such a demand is made, this permit shall be final and binding.
This permit is not transferable except after notifying the Division of Water Quality. The Division may
require permit modification, or revocation and re -issuance. Please note that this permit does not affect
your legal obligation to obtain other permits required by the Division of Water Quality, the Division of
Land Resources, the Coastal Area Management Act, or other federal or local governments.

In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful standards
and regulations promulgated and adopted by the North Carolina Environmental Management
Commission, and the Federal Water Pollution Control Act, as amended,
Caswell County Schools —Board of Education (BOE)
is hereby authorized to discharge wastewater from a facility located at the
South, Elementary School Water Treatment Plant (WTP)
8925 NC Hwy 86 South, Mebane.
Caswell County
to receiving waters designated as an unnamed tributary of Hyco Creek within the Roanoke River Basin
in accordance with effluent limitations, monitoring requirements, and other conditions set forth in Parts
I, II, III, and IV hereof.
This permit shall become effective on August 1, 2012.
This permit and the authorization to discharge shall expire at midnight on March 31, 2017.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked, and as of this issuance, any previously issued permit bearing this number is no longer effective.
Therefore, the exclusive authority to operate and discharge from this facility arises under the permit
conditions, requirements, terms, and provisions described herein.
Caswell County Schools —Board of Education (BOE)
is hereby authorized to:
1. continue to treat filter -backwash wastewater generated during the production of potable water from a well
utilizing green -sand filter water -treatment technology, from facilities including
• caustic soda tank
• potassium permanganate tank
• chlorine -contact chamber /chlorinator
• green -sand filter
• dechlorinator
• holding basin for filter back -wash
• drain line to outfall
located at the South Elementary School Water Treatment Plant, 8925 NC Hwy 86 South, Mebane,
Caswell County and,
2. discharge from said treatment works, at the location specified on the attached map, to an unnamed
tributary to Hyco Creek [stream segment 22-58-1], a waterbody currently classified C within
Subbasin 03-02-05 of the Roanoke River Basin.

Footnotes:
1. E = Effluent; U = Upstream (at least 100 feet upstream from the outfall). D = Downstream
(at least 100 feet downstream from the outfall).
2. In addition to total flow, the Permittee shall record and reportflow rate and flow duration on
monthly Discharge Monitoring Reports (DMRs).
3. Discharge shall not cause receiving -stream turbidity to exceed 50 NTU. If the natural
background turbidity exceeds 50 NTU, this discharge may not cause receiving -stream
turbidity to increase.
4. Total Residual Chlorine (TRC) - The Division shall consider all TRC values reported
below 50µg/L to be compliant with this permit. However, the Permittee shall continue to
record and submit all results reported by North Carolina -certified test methods (including
field certified), even if these results fall below 50µg/L.
5. Total Fluoride to be monitored only if filters are back washed with fluoridated finish water.
Conditions:
1. All samples shall be collected during the backwash cycle.
2. The Permittee shall discharge no floating solids or foam.

Correspondence with the Permitee — the Application for renewal indicates no changes to wastewater treatment
facilities since last renewal other than adding dechlor.
Proposed Renewal - Changes from Previous Permit — the Division will revised the permit and update the
effluent page to reflect the Water Treatment Plant (WTP) Permitting procedures 2009
Total Iron, Total Zinc, and Total Manganese data are available for Reasonable Potential Analyses (RPA) — all
three show reasonable potential to exceed respective water -quality standards (see attached RPA). Total
Manganese monitoring is required for greensand technology, but it remains as "monitoring only, no limit."
Although Total Manganese past data do show RP to exceed the Human Health standard of 200µg/L (MAX
predicted = 4,032µg/L), this standard applies to Class WS waters only. There are no other parameters of
concern.
Toxici - Green sand filter systems do not require Whole Effluent Toxicity (WET) testing.
Compliance - This facility has an excellent compliance record (see attached).
