Division personnel have reviewed and approved your application for renewal of
the subject permit. Accordingly, we are forwarding the attached NPDES discharge
permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the
U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently
amended).
The final permit includes no major changes from the draft permit sent to
you on July 18, 2007.
If any parts, measurement frequencies or sampling requirements contained in
this permit are unacceptable to you, you have the right to an adjudicatory hearing
upon written request within thirty (30) days following receipt of this letter. This request
must be in the form of a written petition, conforming to Chapter 150B of the North
Carolina General Statutes, and filed with the Office of Administrative Hearings (6714
Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is
made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the
Division. The Division may require modification or revocation and reissuance of the
permit. This permit does not affect the legal requirements to obtain other permits
which may be required by the Division of Water Quality or permits required by the
Division of Land Resources, the Coastal Area Management Act or any other Federal or
Local governmental permit that may be required.

All previous NPDES Permits issued to this facility, whether for operation or
discharge are hereby revoked. As of this permit issuance, any previously issued
permit bearing this number is no longer effective. Therefore, the exclusive authority
to operate and discharge from this facility arises under the permit conditions,
requirements, terms, and provisions included herein.
The Caswell County Schools are hereby authorized to:
1. Continue to operate a well water treatment system that discharges backwash

- The Town Manager is no longer in employment there, and they have not hired a replacement. Send the
permit to Dan Printz, Mayor instead. In fact, it's possible that they did not receive your draft, so you
might want to re -send. And it would be good to send a copy to Dennis Hodge at R & A Labs as well,
since they are the operators. His fax number is (336-996-0326).
SOUTH ELEMENTARY SCHOOL
- My comments are the same as they were when we spoke on the phone. This is such a small discharge
that it seems needless for a sample twice a month or even monthly. There is quite a significant distance
from the discharge point into Hyco Creek. When I last spoke to Mr. Hatchett, he was going to stop the
water softening process (they only add it because of ease on the kitchenware) because it would be too
expensive for them.

Discussed Caswell County Elementary School Permit renewal. Softener addition had
made this a complicated permit. Informed Jenny of all information obtained in
conversations with contacts at Caswell County Schools. I am still waiting on call back to
discuss Softener operation with serviceman. Suggested Jenny contact Mr. Hatchett about
possibility of removing Softener, he had mentioned this in a previous call to me. Jenny
stated she would call Mr. Hatchett.

Mr. Hatchett call in response to his conversation with Jenny Freeman. I explained the
impact of the Softener on the permit requirements and if he was going to remove the
Softener then it simplify the permit. I did explain that the renewal permit just for the
Greensand Filters was still going to require additional sampling and analyses. But it is
much less than if the Softener is part of the treatment process.
Mr. Hatchett agreed that the Softener would be removed. I explain that the draft renewal
permit would be for the Greensand Filters only. Also, I would document this phone
conversation and agreement for our file.

Thanks for the quick phone response to my email questions on the South
Elementary School well water treatment system. I believe my remaining
questions will be sufficiently answered by Bob Troy as we discussed.
As I reviewed my notes I want to be sure I correctly documented what the
water flow value on your submitted monthly reports represents. According
to my notes the value is recorded daily from a meter that totalizes the
well production in gallons going to the treatment system /storage tank.
On some days there are 110" gallons because no additional water from the
well is needed. I am sorry about my persistence but I want be sure this
is documented correctly.

In response to the email sent yesterday Mr. Hatchett call me to review the questions.
He did not know the softener had a backwash cycle. It is piped such that there is no open
point for the backwash. It uses a timer which can be operated manually. There is not a
good service manual on file at Caswell County on the softener unit as first thought. We
agreed that in two weeks when the serviceman for the softener, Bob Troy with Driller
Services, visited the site he would call and he could answer my questions.
The greensand filter uses a timer which can be activated manually. This is done when
Pace takes the currently required quarterly sample. The backwash waste line exit the filter
does not extend all the way to the existing floor drain. Thus, it is easy to collect the
sample.
Mr. Hatchett was concern about the softener causing significant changes to the permit. In
fact he was of the opinion that it could be removed since it actual value to the school
operation was in question. I explained that the greensand filter was going to increase his
sampling requirements regardless. The primary issue with the softener at this point was
that it did contribute to the waste stream but it was excluded in the current logistics of the
sampling process which we did need to change under the renewal permit. I mentioned it
may be something that could be accommodated by adding a sample well on the waste
line existing the well house assuming it collected all backwash waters. In any case I
suggested he wait until we had issued the daft permit for comment and then we could
better address the effect of the softener.

Mr. Hatchett,
The assistance by your staff has been appreciated. The current treatment system in
service at the South Elementary School has changed since the last permit review. This
has resulted in the additional phone calls and request for more information. I still
need to clarify a few more details about the current system. Currently you are
required to take grab sample of the backwash water every quarter. Pace Analytical
has provide the monitoring values listed in your reports to us. Who, how, and where
are the quarterly grab sample taken? I assume the person taking the sample is there
when the Greensand system is backwashing. How is the Greensand Filter backwash
initiated for sampling purposes or is it? Based on the information provided by Steve
the Softener Unit regenerates several times during the week but I get the impression
the backwash has not been sampled. Can a grab sample of the Softener backwash be
taken? How is the Softener Backwash initiated? Can both the Greensand Filter and
Softener Unit be backwash at the same time thus producing a mixed backwash stream
that can be sampled? Does the past flow values provided in the monthly reports to us
reflect all the backwash flows from both the filter and softener? In the future both
flows need to be accounted for in the monthly report.
Your assistance in addressing these questions is needed to continue to move your
permit renewal review process forward. If you have any questions feel free to

Called and talked with Mr. Yarboro ?? about softener system. All the water that goes
through the green sand filter goes through the softener. The brand is Treatrite Water
System model unknown. He uses 350# of salt per week. Quarterly grab samples does not
include softener backwash. Mr. Yarboro to call back with model number tomorrow.

Called and asked for Mr. Jerry Hatchett, name on application. He was not in but did
discuss missing details with Sherry McGowan. She requested I give details and had a
copy of the application so she was able to follow my questions. I asked (1) about the salt
tank (schematic) and its purpose since no water softener were shown or mentioned in the
permit (2) how is the waste water flow (backwater) handled after the pump house since
they did supply a sludge plan (3) where is the grab sample currently taken (4) it appears
items 5 and 15 on the application may be incomplete depending on the missing details.
She stated she would pass this on to Mr. Hatchett and ask him to return my call.
Return call: 2:30 PM
First return call was a vm from a Jerry (I was out of the office). Jerry called a second
time (before I retrieved the vm) in response to my questions and was able to resolve the
issues I raised. He is faxing revised information to my attention. There is in fact a
greensand filter and softener tank. Grab samples are taking on the backwash line. The
flow rate shown on the application is the sum of both backwash from the two tanks
Suspect that only the greensand backwash is sampled.
Received and reviewed fax and revised documentation.

Mr. Hatchett gone for the day. Left vm for him to call me tomorrow. Still not clear where
softener unit fits into flow. Also, want to know if only greensand filter taken in grab
sample.

monitoring device or method required to be operated or maintained under Article 21 or regulations of the
Environmental Management Commission implementing that Article, shall be guilty of a misdemeanor punishable
by a fine not to exceed $25,000, or by imprisonment not to exceed six months, or by both. (18 U.S.C. Section
