On April 19, 2022, Ori Tuvia of this Office conducted an inspection at the subject facility. This
inspection was conducted as a Compliance Evaluation Inspection (CEI) to ensure compliance with
permit requirements and conditions. It was noted that no discharge has been reported during the
12-month (March 2022 through February 2023) compliance history review. At the time of
inspection, the subject facility appeared to be well maintained. We wish to thank you for your
assistance regarding this inspection.

Permit is for the continued operation of existing wastewater treatment facility that consist of a 0.5
MGD retention pond that discharges via Outfall #001 and stormwater containment berm that
discharges via Outfall #002. Discharge of boiler blowdown, water softener blowdown and stormwater
is via Outfall #001. Discharges are to an unnamed tributary of Grants Creek, classified as Class C
waters, in the Yadkin Pee -Dee River Basin.
Review of the past 12-months (March 2023 to February 2023) compliance history faclity did not
report any flow at discharge. At time of inspection retention pond had 2-5 foot of free board.
It is recommended that facility investigate removal of waste stream, boiler blowdown and water
softener blowdown, and direct flows to the City of Salisbury sewer collection system. Stormwater could
then be permitted under a Stormwater Permit for the subject site.
