Modifications to the subject Attachments are as follows: ➢ The Division has added Tyson —Danville, VA as an approved residual source, with an approved annual dry tonnage of 1,300 dry tons per year (DT/yr). ➢ The Division has removed Innospec Active Chemicals — Salisbury WWTF (Permit No. WQ0001077) as a residual source. ➢ The Division has modified the approved annual dry tonnage for Dobson Plant (Permit No. NC0006548) from 1,300 DT/yr to 750 DT/yr. ➢ The Division has added Field NC-WK-MF-16 (Parcel No. 4818-65-7884) in Wilkes County, at 48.8 acres. The modified Attachments A and B are effective from the date of certification until November 30, 2027, shall replace Attachments A and B of Permit No. WQ0038087 issued on February 4, 2021, that the Division previously certified on April 25, 2022, and shall be subject to the conditions and limitations therein, as well as those specified in the above -referenced permit. Please replace Attachments A and B of Permit No. WQ0038087 that the Division certified on April 25, 2022, with the modified Attachments A and B. The Permittee shall submit a renewal application no later than June 3, 2027. Maximum Dry Tons per Year is the quantity of residuals approved for land application from each permitted facility. 2. Analyses to demonstrate that the residuals are non -hazardous (i.e., TCLP, ignitability, reactivity, and corrosivity) as stipulated under pennit Condition IV.2. 3. Testing of metals and nutrients as stipulated under permit Condition IV.3. 4. Analyses of the pathogen and vector attraction reductions as stipulated under permit Condition IV. 4. 5. Class B non -biological residuals are exempt from meeting the pathogen and vector attraction reduction requirements in Condition IVA, provided the Permittee does not mix them with residuals generated during the treatment of domestic wastewater, the treatment of animal processing wastewater, or the biological treatment of industrial wastewater. The Permittee shall treat and process Class B non -biological residuals in a manner as not to meet the definition of `Biological Residuals" as defined in 15A NCAC 02T. 1102(6). 6. Mathis Properties, LLC and Terra Renewal Services, Inc. recorded an easement agreement for the installation and operation of this storage facility in the Wilkes County Register of Deeds (DB: 01322 / PG: 0186). A site map for this facility is in Figure 32. 8. The Permittee may temporarily store residuals from the Division -approved residual sources in the above table in this structure. The Permittee shall subtract any residuals placed into this storage structure from the Division -approved residual source's maximum dry tonnage per year. 9. The Permittee conduct representative samples of comingled residuals immediately prior to land application events to ensure that the regrowth of pathogens has not exceeded the pathogen limits for Class B biological residuals established in 15A NCAC 02T .I106(b)(1).
