Subject: Modification of NPDES Permit NC0066397

Cleveland County
The Division issued the subject permit on October 9, 2003. That permit contained two typographical errors
regarding the permitted flow at the facility: 0.066 MGD was written where 0.0066 MGD should have been.
This modification corrects the two typographical errors. No other changes to the permit have been made.
Please find enclosed the revised pages, which should be inserted into your permit. The old pages may then be
discarded. All other terms and conditions contained in the original permit remain unchanged and in full effect. This
permit modification is issued under the requirements of North Carolina General Statutes 143-215.1 and the
Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency.
If any parts, measurement frequencies or sampling requirements contained in this permit modification are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days
following receipt of this letter. This request must be a written petition conforming to Chapter 150B of the North
Carolina General Statutes, filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North
Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby revoked. As of this
permit issuance, any previously issued permit bearing this number is no longer effective. Therefore, the exclusive
authority to operate and discharge from this facility arises under the permit conditions, requirements, terms, and
provisions included herein.
The Cleveland County Board of Education is hereby authorized to:
1. Continue to operate an existing 0.0066 MGD wastewater treatment facility with the
following components:
♦ Septic tank
♦ Recirculating surface sand filter
♦ UV disinfection system
The facility is located off NCSR 1518 (School House Road) in Casar at the Casar
Elementary School WWTP in Cleveland County.
2. Discharge from said treatment works at the location specified on the attached map into
an unnamed tributary to Crooked Run Creek, a class C stream in the Broad River Basin.

During the period beginning on the effective date of this permit and lasting until expiration, the Pernuttee is
authorized to discharge from outfall 001. Such discharges shall be limited and monitored by the Permittee as

Footnotes:
1. Upstream: at least 50 feet upstream from the outfall. Downstream: 0.1 miles downstream from the outfall.
2. The daily average dissolved oxygen effluent concentration shall not be less than 6.0 mg/L.
3. This limit applies only if chlorine is used for disinfection in place of the existing UV system.
4. The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units.
There shall be no discharge of floating solids or visible foam in other than trace amounts.

Subject permit was reissued on 10/9/03, effective 11/1/03. Supplement
to Permit Cover Sheet and Limits/Monitoring Requirements pages indicate
hydraulic capacity and Flow limit is 0.066 MGD. They should indicate
0.0066 MGD. MRO requests that a correction be issued.
