Division of Water Resources INSTRUCTIONS FOR FORM CSA 04-16&SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant
Division Policies. Failure to submit all required items will necessitate additional processing and review time.

General — When submitting an application to the Municipal Permitting Unit, please use the following instructions as a
checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided
boxes will help produce a quicker review time and reduce the amount of requested additional information.
The Applicant shall submit one original and one copy of the application and supporting documentation.
The copy may be submitted in digital format.
A. Cover Letter
El Submit a cover letter listing all items and attachments included in the permit application package
B. No Application Fee Required
➢ No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit

C. System-Wide Wastewater Collection System (FORM: CSA 04-16) Application:
® Submit the completed and appropriately executed System-wide Wastewater Collection System (FORM: CSA 04-
16) application. Any unauthorized content changes to this form shall result in the application package being
returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long
as the attachments are numbered to correspond to the section and item to which they refer.
❑ If the Applicant Type in Section I.3 is a Privately-Owned Public Utility, provide the Certificate of Public
Convenience and Necessity(CPCN)from the North Carolina Utilities Commission demonstrating the Applicant is
authorized to hold the utility franchise for the area to be served by the wastewater collection system,or
❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an
application for a franchise has been received and that the service area is contiguous to an existing franchised area
or that franchise approval is expected.
❑ If the Applicant Type in Section I.3 is a corporation or company, provide documentation if it is registered for
business with the North Carolina Secretary of State.

RESOLUTION BY THE TOWN COUNCIL OF WARRENTON, NORTH CAROLINA
Adopting the Updated Town of Warrenton's
10-Year Wastewater Capital Improvements Plan
WHEREAS, The Town of Warrenton operates and maintains a wastewater collection system,
and
WHEREAS, The Town of Warrenton has developed a 10-Year Wastewater Capital Improvements
Plan to identify needed capital improvements and their costs,
NOW THEREFORE BE IT RESOLVED, BY THE TOWN COUNCIL OF THE TOWN OF WARRENTON
That, the Town of Warrenton Town Council hereby adopts the updated 10-Year Wastewater
Capital Improvement Plan as a guidance document for pursuing capital improvement funds to
keep the wastewater collection system in good operating order, and
That town staff, under the direction of the Mayor and Town Council, shall regularly review and
update the Wastewater Capital Improvement Plan to ensure compliance with all federal, state
and local regulations.

CONTINGENCY PLAN
Submersible Pump
• Check Breaker
• Reset Breaker
• Turn pump to hand to see if it runs, if it runs operate manually and
call and electrician to fix controls ASAP.
• If it does not run at all, call an electrician for repair ASAP.
• If pump runs but trips out call for crane and assistance to pull pump
and check for possible stopped up impeller.
• If you cannot quickly find the problem, call an electrician to come and
fix ASAP.
• If electrician finds a problem with pump, have pump pulled and
carried for out for repair ASAP.
• If both pumps go out call for vac truck immediately while you try to
get station going, call an electrician pump service ASAP. Use vac
trucj to pump around until you get station going again. Set up bypass
pump if station is going to be down for extended time or if pump truck
can't keep up if possible.
Above Ground
• Check Breaker
• Reset Breaker
• If motors don't run, switch to hand/manual to see if it runs, if not call
an electrician ASAP.
40 If motor does run, run manually and call an electrician ASAP.
• If motors will not run manually try to reset breaker, if it doesn't reset
call an electrician ASAP.
• If motor runs but pump will not pump, backwash pump with other
pump, reprime pump, if pump does not start working call pump repair
tech immediately.
• If both pumps go out call for vac truck immediately while you try to
get station going, call an electrician pump service ASAP. Use vac
truck to pump around until you get station going again. Set up bypass
pump if station is going to be down for extended time or if pump truck
can't keep up if possible.
