Dear Ms. Hamlin:
The Division has received and reviewed your request to modify the subject permit. The
Division does not object to your request. This modification is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended). Insert the attached pages into your permit and discard the old pages.
This modified permit includes the following changes from your current permit:
➢ The facility component list has been updated.
➢ Monitoring frequencies have been updated in Section A. (1.) for all parameters except
Ammonia Nitrogen, due to the current facility classification per 15A NCAC 02B .0508.
If any parts, measurement frequencies or sampling requirements contained in this
modification are unacceptable to you, you have the right to an adjudicatory hearing upon written
request within thirty (30) days following receipt of this letter. This request must be in the form of a
written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with
the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714).
Unless such demand is made, this decision shall be final and binding.
This permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by any other Federal, State, or Local

During the period beginning on the effective date of this permit and lasting until

1. The Division shall consider all effluent TRC values reported below 50 µg/L to be in compliance with
the permit. However, the Permittee shall continue to record and submit all values reported by a
North Carolina certified laboratory (including field certified), even if these values fall below 50 µg/L.
2. The Permittee shall submit discharge monitoring reports electronically using the Division's eDMR
system [see A. (3.)].

Hey Emily,
Thanks for the call today. For these two facilities we are just trying to get monitoring frequencies in line with a Class II
facility. They are both currently sampling under frequencies for a Class I facility. Under 0213, Minimum Monitoring
Requirements for Domestic Wastewater and Other Facilities Discharging Primarily Domestic Wastewater the following
frequencies are listed;

Lynnbrook and Pavilion are both Class II facilities in accordance with 08G.0302 classification (activated sludge facilities
<0.5 MGD flow). I know the flows are low for both of these facilities; however, sometimes these are our most
problematic facilities so I encourage we at least ask for the minimum sampling requirements for the assigned
classification.
This would increase BOD, TSS, Fecal and pH to weekly and NH3 to 2/month for both facilities. Additionally, Lynnbrook is
only sampling temperature 2/month (normal monitoring is weekly for both Class I & 11).

ARO permit comments
John,
I hope you are doing well. I wanted to bring to your attention an issue where Mikal of my staff had provided permit
review comments that were never incorporated into the permit nor an explanation provided as to why. The facilities are
NC0085294 and NC0071005. Comments were provided by Mikal on May 15`^ related to the fact that NC0085294 had
significant changes to treatment components (no longer a sand filter) and NC0071005 had monitoring listed for a Class I
facility. Both these permits were issued on July 9" with no change.
Please let me know what we can do to see that we have accurate permits and staff comments are either incorporated or
staff receive a response as to why.

In accordance with North Carolina General Statute § 90A-37, the Water Pollution Control System
Operators Certification Commission is required to classify all water pollution control systems.
The Commission has determined that the subject facility is classified as a Grade 2 Biological Water
Pollution Control Treatment System, based upon an upgrade of the facilities to an extended
aeration activated sludge process which was authorized in late 2013 and is now operational.
As required by 15A NCAC SG .0201 (1) and (2) and your permit, you must designate a Grade 2 or
higher Biological Operator in Responsible Charge (ORC) and a Back-up Operator with at least a
Grade 1 Biological certification. Please complete and return the enclosed ORC Designation Form.
