Dear Mr. Minter:
In accordance with the application for a discharge permit received on June 29, 1998, the Division
is forwarding herewith the subject state - NPDES permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
North Carolina and the U.S. Environmental Protection Agency dated December 6, 1983. .
Please be aware of the following changes in this NPDES permit renewal:
• The sampling frequency for pH was changed from Weekly to 2/Month.
• The permit expiration date was changed to July 31, 2003.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the office of Administrative Hearings,
Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such a demand is made, this
permit shall be final and binding.
Please take notice that this permit is not transferable. Part II, EA. addresses the requirements to be
followed in case of change in ownership or control of this discharge.
This permit does not affect the legal requirements to obtain other permits which may be required
by the Division of Water Quality or permits required by the Division of Land Resources,
Coastal Area Management Act, or any other Federal or Local governmental permits may be required.

to receiving waters designated as Skyuka Creek in the Broad River Basin
in accordance with effluent limitations, monitoring requirements, and other conditions set forth in
Parts I, II, III, and IV hereof.
This permit shall become effective January 1, 1999.
This permit and authorization to discharge shall expire at midnight on July 31, 2003.

facility consisting of diffused aeration, clarifier, sludge holding, skimmer, sludge
returns, and effluent chlorination, located at Lynnbrook Estates Wastewater Treatment
Plant, NCSR 1135, Columbus, Polk County (See Part IIIA of this permit), and
2. Discharge wastewater from said treatment works at the location specified on the
attached map into Skyuka Creek (via outfall 001), which is classified C waters in the
Broad River Basin.

Summary
Minor residential facility discharges 100% domestic wastewater to a 0.009 MGD
extended aeration package plant to Skyuka Creek in the Broad River Basin. Skyuka
Creek is classified C waters, and is not listed as a 303(d) impaired waterbody.
Permit Changes. Previous NPDES permit modification (dated May 1995) allowed
monitoring frequency to be reduced from weekly to 2/Month, based on good compliance
history and small waste flow. Propose changing pH monitoring frequency from weekly
to 2/Month to maintain consistency with previous permit modification.
DMR Data. Only 2 daily max FC violations noted over past 2 calendar years(1996-97).
Staff Report. The file does not contain an ARO staff report. Compliance Evaluation
Inspections conducted in1996 and 1997 found the facility to be operating very well and
in compliance with permit limits.
WLA. The last WLA was conducted in 1994 using the current design flow (0.009
MGD. Secondary limits were recommended for BOD/TSS. FC limits were
recommended based on dilution < 331:1. Mass balance did not indicate need for TRC
or NH3 limits.
