Per 15A NCAC 02'F-0105(c)(6), directly related environmental permits or certification applications must be being prepared,
have been applied for, or have been obtained. Issuance of this permit is contingent on issuance of dependent permits (erosion
and sedimentation control plans, stormwater management plans, etc.).

On behalf of Nash County, we are submitting one (1) original and one (1) copy of the following
documents related to the above referenced project for your review and approval:
• Application Fee ($480.00)
• Sewer Fast -Track Application (FTA 04-16)
• Town of Middlesex, sewer acceptance
• Cover Letter Narrative (see paragraph below)
• USGS Topographic Map showing proposed utilities
• Aerial map showing general project area
This project will install approximately 5,004 linear feet of 8" PVC gravity sewer, approximately
487 linear feet of 8" ductile iron gravity sewer main, 1,661 feet of 4" PVC forcemain, 50 linear
feet of 4" ductile iron force main. 130 gpm pump station, and removal of a 100 gpm pump
station. The project extends sewer service to additional portions of Middlesex Corporate Centre
(MCC) and will handle flows from existing customers along US264A outside of MCC. This
project is currently a 0 flow permit request as there are no tenants within MCC operating at this
time. Future tenants will apply individually for sewer flows during the design process.
The Town of Middlesex will own and maintain the proposed gravity sewer after construction.
Ownership change form (from Nash County to Town of Middlesex) as well as a letter stating this
sewer extension will be maintained by the Town of Middlesex is attached. All new sewer
improvements have been designed in accordance with the 15A NCAC 02T requirements.

Project is a zero -flow project. The gravity sewer is proposed to serve future tenants when they
occupy existing MCC sites.
FTA 06-21 VI. General Requirements No. 8
As of February 2023 there are two (2) parties pursuing the purchase of lots within the park.
December of 2023 is the earliest possible time flows may be requested for service via the
proposed infrastructure requested with this permit.
FTA 06-21 IX. Setbacks and Separations No. 3 and 5.
Project results in less than 0.1 acres of temporary wetland impacts. USGS map shows a blue
lined stream where a small wetland currently exists. Area has no stream geomorphology,
hydrology, or biology present. Wetland soils, hydrology, and plants exist. No Preconstruction
Notification is required and these impacts are allowable as promulgated in the general conditions
of USACE NW Permit 58 and NCDEQ WQ 4276. No streams or stream buffers will be
impacted by this project. Temporary impact location can be found on sheet C-1.06, from station
25+30 to 26+00.

Regarding the Middlesex Corporate Centre Phase B project. We have 1 customer requesting sewer availability by
12/31/2023 (approximately 6,250 GPD) and two other customers requesting sewer availability by Fall of 2024
(approximately 1,100 GPD). These customers are primarily warehouses with distribution businesses, flows are from
employee restrooms.
One of these customers has closed on property purchase, the remaining customers have closing dates this summer.

Following up on our conversation yesterday regarding the Middlesex Corporate Centre Phase B project;
please provide some information detailing future plans for the area to be served by the zero -flow gravity sewer line that
is included in the permit application.
Specifically, per Section VI.8. Of the permit application form "Please provide supplementary information indicating the
approximate timeframe for permitting upstream sewers with flow."
As I mentioned yesterday, we don't typically permit zero -flow gravity lines unless they are going to be put into use under
a separate permit application in the very near future.
That being the case, we need documentation that this proposed gravity line agrees with this description.
