Dear Mr. Deemer:
A letter of request for an Authorization to Construct was received October 3,
1988, by the Division and final plans and specifications for the subject project
have been reviewed and found to be satisfactory. Authorization is hereby granted
for the construction of a 1.0 MGD wastewater treatment facility consisting of a
septic tank, five (5) 40 sq.ft. shaker screens, a 30,000 gallon DAF unit with
chemical addition, a 1.2 million gallon capacity aerated surge lagoon, a 3.9 million
gallon storage lagoon, a 92-acre terraced overland flow system, dual 700 GPM
irrigation pumps,a parshall flume and gas chlorination system.
This Authorization to Construct is issued in accordance with Part III paragraph
B of the NPDES Permit No. NCO072575 issued April 4, 1988 , and shall be subject
to the following conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications, and other supporting data.
2. The Permittee must employ a certified wastewater operator in accordance with
Part III paragraph C of the referenced permit.
3. The sludge generated from these treatment facilities must be disposed of in
'accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
4. The Raleigh Regional Office, telephone number 919/733-2314 shall be notified
at least twenty-four (24) hours in advance of operation of these facilities.
Such notification to the Regional Supervisor shall 'le made during the normal
office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding
State Holidays.

5. In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the permittee shall take such immediate corrective
action as may be required by this Division, including -the construction of
additional wastewater treatment and disposal facilities.
6. Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer
certifying that the permitted facility has been installed in accordance with the
approved plans and specifications. Mail the Certification to the Permits and
Engineering Unit, P.O. Box 27687, Raleigh, N.C. 27611-7687.
7. A set of approved plans and specifications for the subject project must be
retained by the Permittee for the life of the project.
8. In the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permit -tee shall take such immediate
corrective action as may be required by this Division, including the
construction of additional or replacement wastewater treatment or disposal
facilities.
9. A suitable year round vegetative cover shall be maintained.
10. Freeboard in the lagoons shall not be less than two (2) at any time.
Prior to beginning waste disposal operations, five (5) monitor wells, two (2)
-- upgradient and three (3) downgradient, must be installed to monitor groundwater
quality. One (1) upgradient and one (1) downgradient monitor well shall be
installed in the vicinity of the storage ponds. One (1) upgradient and two (2)
downgradient monitor wells shall be installed in the vicinity of the overland
flow system. The location and construction details for these wells must be by
approval of the Raleigh Regional Office, from which a well construction permit
must be obtained.
12. The monitor wells must be sampled initially after construction (and prior to
waste disposal operations) and thereafter every March, July, and November for
the following parameters:
NO3 pH
TOC Water Level
TDS Chloride
Total Coliforms SO4
Orthophosphate
The measurement of water level must be made prior to sampling for the
remaining parameters.
The results of the sampling and analysis shall be sent to the N. C. Division
of Environmental Management on Form GW-59 (Compliance Monitoring Report
Form) every April, August, and December.
13. Prior to beginning waste disposal operations, the observation wells which were
/'IA abandoned by backfilling with clean local fill material shall be properly
abandoned according to procedures outlined in 15 NCAS 2C, "Well Construction
Standards."
14. Any additional groundwater quality monitoring, as deemed necessary by the
Department, shall be provided.
15. The surge lagoon and storage lagoon shall have liners of natural material, at
least one (1) foot in thickness at all locations, and have a maximum hydrualic
conductivity of 1 x 10-6 cm/sec when compacted. Following installation and
inspection of the lagoon liners and prior to waste disposal operations,
vertification of the liner's compliance with permeability and thickness
specifications should be provided to the Division of Environmental Management
Groundwater Section, by the project engineer.
