The Division of Energy, Mineral, and Land Resources (DEMLR), received a complete, Stormwater Management Permit
Authorization to Construct(ATC)Application for the subject project on September 8, 2023. Staff review of the plans and
specifications has determined that the project, as proposed, will comply with the Stormwater Regulations set forth in Title
15A NCAC 2H.1000-1062 rules.We are forwarding ATC Permit No.SWA000226, dated October 2,2023,for the construction
of the subject project and the stormwater system. This permit shall be effective from the date of issuance and shall be subject
to the conditions and limitations as specified therein, and does not supersede any other agency permits that may be required.
The original signed permit package and plans are being mailed to the permittee, The Reverend Nicholas Newman, at the

Reminder: Please be aware that per 15A NCAC 02H .1044(2), the Erosion &Sediment Control (E&SC) permit for this site
cannot be closed out until the as-built stormwater plans for this project are approved by our office.

review queue on it). Please upload all documents that you mailed into our office through the link provided below. Be
sure to choose the Central Office as the Primary Contact office.

IMPORTANT NOTE: Please ensure that each file is uploaded under its appropriate file type. Additional file types can be
entered into the electronic form by clicking on the "Add Another Post-Construction File Upload" link directly below the
"State Stormwater Information Uploads" section of the form. YOU MUST CLICK ON THE"ADD ANOTHER POST-
CONSTRUCTION FILE UPLOAD"TO ADD ADDITIONAL FILE TYPES TO THE SUBMISSION. Merely changing the file type
dropdown as you upload the various files (without adding additional file uploads)will upload all of the files as the last
file type selected,the submission will get flagged during review, and you will need to resubmit the files. Please do not

We received your application for a new Fast-Track post-construction stormwater permit on 9/5/23. At this time, the
submission is not administratively complete and cannot be accepted for review in its current condition. Please provide
the following submittal items within 30 calendar days (by close of business on 10/5/23) so that we may accept the
project and begin the review process, otherwise, the project will be returned as administratively incomplete.
• 2x hard copies and 1x electronic copy of the Application with the following corrections made (These items are
required per 15A NCAC 02H .1043(3)(a) and per Section F of the Application):
• Section B, 3—Please provide accurate information concerning the physical location (street address) of
the project. This is required per 15A NCAC 02H .1043(3)(a)(ii). Note:The address listed for this project is
7801 NC HWY E, the HWY number seems to have been left off.
• Section E, 4- Please use the information for the first named surface water("Adams Creek", "C" &"13-
17-11-17")for the receiving stream name, classification and index number.This is required per 15A
NCAC 02H .1043(3).
• Section E, 5—Please list the total surface water area for the project.This is required per 15A NCAC 02H
.1043(3).
• Section G—Please provide the applicant's/financially responsible party's original, notarized and sealed
signature in this section of the Application. NOTE:The signatures we received were not original.
• Section H—Please provide the design professional's original, notarized and sealed signature in this
section of the Application. NOTE:The signature we received were not original.
NOTE:This is an administrative completeness review, not a detailed engineering review, there may be other issues that
may be brought up during the engineering review.
If these items are received within the stipulated timeframe, we can accept the project and begin the review process.
Once accepted, reviews under the Fast-Track permitting process can take up to 30 calendar days per statute.
