This re -inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in Non -discharge Permit No. WQ0022725. A summary of the findings and comments noted during the Inspection are provided in the enclosed copy of the inspection report. The Compliance Sampling inspection was conducted by Division of Water Resources staff from the Washington Regional Office. The following violation(s) were noted during the inspection: Inspection Area Description of Violation Treatment RIters Tertiary filters are not operational. Treatment How Flow meter was not calibrated within the last year. Recording capabilities were not Measurement -Effluent worldng and not sure whether the flow meter Is accurately worldng. Record Keeping No records were provided. Operator was Informed that a follow up Inspection was to be conducted and to bring all records for the Inspection. No records were provided. North Carok a VU9 Inspection Area Description of Violation Treatment Tertiary Filters, EQ pumps, return pumps, uv unit, turbidity meter were all nonoperational. Treatment Disinfection The UV Intensity panel was not functional. Only two lights were on. In addition, the issues below must also be addressed: Compliance Issue(s): Redaimed Reuse: 5 day upset tanks were full and not ready to accept upset water from treatment system. Reclaimed Reuse: Irrigation requires field calibration. Wastewater Treatment Meters: All meters require annual calibration from outside contractor. A Notice of Violation/Intent to Issue Civil Penalty is being issued for the noted violation of North Carolina General Statute (G.S.) 143-215.1 and the facility's Non -discharge Permit. Pursuant to G.S. 143-215.6A, a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against any person who violates or fails to act in accordance with the terms, conditions, or requirements of any permit issued pursuant to G.S. 143-215.1. If you wish to provide additional information regarding the noted violation, request technical assistance, or discuss overall compliance please respond in writing within ten (10) business days after receipt of this Notice. A review of your response will be considered along with any additional information provided. You will then be notified of any civil penalties that may be assessed regarding the violations. If no response is received in this Office within the 10-day period, a civil penalty assessment may be prepared. Remedial actions should have already been taken to correct this problem and prevent further occurrences in the future. The Division of Water Resources may pursue enforcement action for this and any additional violations of State law. If the violations are of a continuing nature, not related to operation and/or maintenance problems, and you anticipate remedial construction activities, then you may wish to consider applying for a Special Order by Consent. Robert Tankard and Sarah Toppen were present for the Division and David and Michelle Pharr were present for the Slash WWTP. DWR staff arrived around gam and began to pull effluent samples. The operators arrived and the Inspection resumed. Inspection started at the head of the plant. Equalization pumps were not operating. Both pumps were sitting on catwalks. Could not acknowledge whether the mixers were present and operating. Operator had put in a bypass from the eq basin to the aeration basin. The eq basin had lots of grease in the tank. Nothing had changed since the last inspection. Dlgestor was full of solidstwastewater. Did not have the two foot freeboard in the tank. Operator stated no alarms for this tank. Please check on this issue. Air was not on in the aeration basin. The operator then tweaked the controls and the air started. Clarifiers appeared to be working. Asked the operator to measure the sludge blanket and operator could not because he had not brought a sludge judge. Tertiary filters back wash did not work Operator stated that the filters did not work. The 11/19/2020 and the 05/2023 inspections also indicated that the filters back wash did not work , UV intensity was at 50 percent. 2 Bulbs were not working. Operator stated he had replacements in the car on the previous Inspection. Only two bulbs were working on the 5/2023 inspection. Turbidity and flow meters were last calibrated on March 16, 2022. Turbidity meter was not functional. Five day storage was completely full of water. On the 5/2023 inspection, the operator was informed that the tanks were cement and were designed to be empty to accommodate upset water. Informed him that the water needed to be sent back to the head of the plant and tanks to be emptied for intended use. Tanks were still full on this site visit Irrigation was not operated on the day of inspection. The WWTP was found to be non compliant A reinspection will take place In thirty days to confirm treatment units are in fact operational. Please have the following corrected within 30 days from the date of inspection: 1. Both equalization pumps repaired and Installed in equalization basin. Bypass lines and pump to be removed from treatment plant. 2. Tertiary filter to be repaired and made operational in auto and manual mode. 3. UV system to be repaired. All four bulbs should be working. 4. Five-day tanks water pumped back to the head of the plant Tanks are to be remain empty unless are needed to meet reclaimed treatment. 5. All Irrigation fields to be field calibrated. Have irrigation operator on site at the time of reinspection to verify operation. 6. Turbidity meter to be repaired by an outside contractor. 7. All meters to be calibrated by an outside contractor that specializes in meter calibration. 8. Reinspection to Include compliance sampling. Split sampling is preferred. 9. All records'to be onsite during next Inspection. Facility was found to be non -compliant A notice of violation will be sent for all violations. DWR received sample results for Fecal Coliform from the lab. Fecal was greater than 2420. Operator was notified to transfer wastewater to the five day upset tanks.
