Larry Wade, P.E. has asked me to check this application for completion as he is on vacation for two weeks, this week and
next.
I need Supplement forms for each bmp: Grass Swale Supplement and the Dry Detention Basin Supplement.
They are found here on our website:

Look for BMP Supplement forms link at bottom left side of webpage.
These are online forms that have automatic calculations built in.
If you could fill these out and then copy and email them to me, that would be acceptable.
I note that you provide some sealed stormwater calculations, but they are not on the NCDENR forms. Please seal these
calcs as well.
We are not allowed to fill out the Supplement forms for you.

Pending review of your calculations and site plan, I can proceed with the processing and issuance of this stormwater
permit as soon as I receive the correct forms.

Thank you for your response regarding the Permit Application for US Army Parking Facility, Cumberland County.
We have reviewed the summary of requested information provided in your letter. Please find included our responses
to your comments from the original submittal. Also, please find included with this letter an attachment with revised
topographic map showing the existing contours, also with a revised exhibit showing the drainage area.

ms response: A signed signature page for the application will be provided as an
enclosure with this response.
1. The design calculations provided were not sealed by a licensed Professional Engineer. Please
have the engineer sign seal and date the calculations.
ms response: The drainage calculations will be signed, stamped and dated by a
licensed Professional Engineer.
3. The plan sheets provided were sealed and signed by the engineer, however; there was no date on
the seal. Please have the engineer date the seal on the plans.
ms response: A licensed Professional Engineer with stamp, sign, date the plan
sheets and they will he included with this response.
4. Provide a topographic map that shows the surrounding site topography that includes any offsite
drainage areas.
ms response: The existing topography has been included on an exhibit that
shows the existing contours and drainage infrastructure that will provide
evidence that the proposed parking lot does not collect any offsite drainage
areas.
5. Provide a drawing that delineates the drainage areas the design calculations are based on.
ms response: The drainage area that the design calculations are based on have
been included within an exhibit being submitted with this response.
6. Provide a soils report by a licensed Soil Scientist describing the soil characteristics of the site that
includes the groundwater elevation at the proposed BMP locations.
ms response: A copy of a soils report that has been compiled by a licensed Soil
Scientist describing the site that includes the groundwater elevation at the
proposed BMP location.
7. Demonstrate the BMP designs meet the minimum requirements of NCAC 15A 02H .1000 and the
current BMP Design manual for 85% TSS removal.
ms response: The BMP design that has been presented includes a grassed
swale, an extended dry detention basin, as well as a level spreader and
vegetated Swale. These three items will provide 35%, 50%, and 40%
respectively thus resulting in design that will meet the 85% removal that is
required for the project.

8. Your submittal included a Financial Responsibility Form and other documents that are normally
required for Sedimentation and Erosion Control Plan approval. Please contact our Fayetteville
Regional Office for these issues.
ms response: Sediment and Erosion Control plan approval will be coordinated
with the Fayetteville Regional Office.
