You said you received our revised plans last week, but I haven't heard about any approvals. Please let us know if you need additional information to proceed with the review and approval. Also, we received a comment from NCDOT about revising our pipe outfall locations and elevations to tie into their future proposed storm structures. We have revised the plans accordingly. Please let us know what information, if any, you need from us based on the changes. I have attached PDF's of the revised sheets. Thank you, W. Director November 16, 2012 Dee Freeman Secretary Subject: Request for Additional Information Stormwater Project No. SW6121004 McDonald's 932-1588, Spring Lake Harnett County The Division of Water Quality Central Office received a Stormwater Management Permit Application for the subject project on October 29, 2012. A preliminary review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: Need letter from LLC, authorizing James M. Kurtz as the signatory, as he is not listed on the NC Secretary of State corporation form as an agent or officer. 2. Some sort of pretreatment must be provided for all infiltration devices. See BMP Design Manual Chapter 16, Major Design Elements number 20, page 16-2, also see site plan map sheet C-5.1. Question: Are you asking for credit from the permeable pavement shown on the site plan? It does not appear that you are. Such credit is not available due to the compacted subgrade, see BMP Design Manual Chapter 18, Major Design Elements, number 12, page 18-3, also see site plan map sheet C-7.0, Vehicular Permeable Paver Section. Please note that this request for additional information is in response to a preliminary review. The requested information should be received by this Office prior to December 16, 2012, or the application will be returned as incomplete. The request must indicate the date by which you expect to submit the required information. The Division is allowed 40 days from the receipt of a completed application to issue the permit. The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A. Please reference the State assigned project number SW6121004 on all correspondence. All original documents must be returned or new originals must be provided. Copies are not acceptable.
