The Division of Water Quality Central Office received a Stormwater Management Permit Application for
the subject project on March 25, 2009; with follow up information on April 1, 2009. A preliminary review
of that information has determined that the application is not complete. The following information is
needed to continue the stormwater review:
�/ 1. Please delineate all wetlands on site, or provide a note on the plans (or in a letter) stating there
are no wetlands on site.
,/2. Information on the seasonal high water table (SHWT) needs to be provided. The soils report
provided does not specifically estimate the SHWT at each of the bioretention cells. At least one
soil boring is required within the foot print of each cell to estimate SHWT. The SFIWT must be
at least 2 feet below the bottom of the cell.
✓ 3. An O&M Agreement was submitted, but not signed and notarized. Please submit a completed
O&M Agreement for the bioretention cells (1 original & 1 copy).
AThe drainage calculations must use the Rational Method, unless you are designing a LID
project. Please revise the calculations accordingly.
/5. For future submissions, please be sure to initial each item in the submittal requirements on page
4 of SWU-101.
6. Please revise the drainage area table in III.8. of SWU-101 to reflect the same numbers shown on
the plans, calculations, and supplement forms. You may submit just the revised page(s) if you
so choose.

7. Some type of pretreatment device (forebay, plunge pool, etc.) is required at each flume into the
bioretention cells. Also, please provide calculations to show that the inlet velocity for grassed
cells is no more than 3 ft/sec (1 ft/sec for mulch/shrub cells).
J 8. The bioretention materials specifications on C-5.2 includes shredded hardwood mulch. What is
the mulch used for in these grassed cells? Please explain and revise as needed.
j9. Sheet L=1 shows shrubs and trees within the bioretention cells, but the cells are designed to be
grassed cells. If they are shrub/tree cells, the planting media depth will need to be increased to
a minimum of 3 feet deep to ensure plant survival. Please explain and revise as needed.
J 10. Please note for future reference: The State storn►water rules do not require peak attenuation of
the 1-yr, 24-hr storm as stated in the narrative. The related requirement that is in the rules, is
that the discharge rate of the storage volume (treatment volume) must be less than or equal to
the pre -development l -yr, 24-hr storm rate. This project meets that requirement.
Please be sure to revise any plans, calculations, and forms that are affected by any changes based on the
above comments. If any plan sheets must be revised, be sure to submit two copies of the revised sheets.
Please note that this request for additional information is in response to a preliminary review. The
requested information should be received by this Office prior to April 20, 2009, or the application will be
returned as incomplete. The return of a project will necessitate resubmittal of all required items, including
the application fee.
If you need additional time to submit the information, please mail, fax, or email your request for a time
extension to the Division at the address and fax number at the bottom of this letter. The request must
indicate the date by which you expect to submit the required information. The Division is allowed 90 days
from the receipt of a completed application to issue the permit.
The construction of any impervious surfaces, other than a construction entrance under an approved
Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement
action pursuant to NCGS 143-215.6A.
Please reference the State assigned project number SW4090301 on all correspondence. All original
documents must be returned or new originals must be provided. Copies are not acceptable. If you have

I left you a voice mail earlier today. Please tali me to discuss this
when you have a chance. Based on a preliminary review, the application
is incomplete. We cannot process your application until, these some of
these are resolved. Otherwise, I'll have to return the application as
incomplete. All these requirements are on page 4 of SWU-101 in the
submittal requirements checklist which was not initialed as required; in
the BMP Manual; in the rules; or in the required items checklist of the
supplement form. '
1. The application and forms must be signed by someone having legal
authority to do so. C. Nadine perry is not in the Sec. of State's
database for this LLC as having signing authority. Another option is to

get a letter from someone who does have signing authority giving the
same•to C. Nadine Perry. I.am technically suppose to return the whole
package due to this, but will hang on to it if you can get this resolved
quickly'. —The narnes,.in the SOS database are Gloria Santona, Matthew H.
Paull, Ralph 'Alvarez,.- and Marcy. Miller.
2. The bioretention 0&M,form was included, but it must be signed by the
owner.
3. The Rational Method must.be used for the calculations. The SCS
method can only be used if it ia;shown to be more conservative than the
rational method, or if it is an LID project.
4. Any wetlands must be shown on the plans or a note must be on the
plans saying none exist on the site. ,
5. The soils report was submitted, but it does not appear to contain
the information required. A boring is required in the footprint of each
bmp, with a field determination of the estimated SHWT. This is based on
visual inspection of the soil, not whether water is seen in the boring.
