Crystal Amschler, Dave McHenry, Mark Davis, and Kevin Barnett were present during the inspection. The following are items that need attention: Section A - Station 45+00 -L- LT (Site 4) - Need measures below exposed soil on slope in order to protect JS. Urgent - Station 65+00 -L- LT (Site 6) - Detail 9C shows rip rap in channel (keyed in). The bed did not appear to have rip rap in it. Rip rap is often necessary to accomodate stormwater flows. Add rip rap per plan or provide justification for why it is no longer needed. Sorry, this was not discussed in the field. Also need to removed trash and debris at bottom of slope. - Station 108+50 -L- LT (Slay Bacon) - Add cover to slopes above headwall. No measures in place to protect JS. Urgent. - Station 114+00 -L- RT (Site 10) - Agencies had concerns about the low water flows at the outlet directed toward the Left Bank Descending. We agreed to monitor after the next high flow event to determine if additional rip rap is needed. - Station 134+50 -L- LT (Site 13) - No erosion control at outlet of 66" bore. Measures are needed. Urgent. We also discussed adding coir fiber to inlet bed when the channel change is completed. Section B - Station 311+00 - 312+00 -L- RT (Site 17) - Mat slopes and fix silt fence at bottom of slope. Urgent - Station 312+00 -L- LT (Inlet of Site 17) - 10 feet of rip rap stabilization was added to banks. Need cover on slopes to meet NPDES cover requirements. Section C - Station 49+50 -L- RT (Site 11) - Inlet work looks good. Benches were built at inlet. - Station 52+00 -L- LT (Site 11) - Add cover to disturbed area next to channel change. - Station 68+00 -L- LT (Site 15) - Rip rap in channel is not keyed in per detail. Recommend choking the rip rap with mix of smaller substrate in order to bring flows above rip rap. Dirt between the JS and silt fence needs to be stabilized. Continue to meet NPDES cover requirements on the project. page: 2 Site Number: 20201371 Owner: Blazer, Keith Inspection Date: 9/27/2023 Inspection Type: Site Inspection (DOT) Reason for Visit: Routine Permit (401 WOO Does the impact(s) match what was approved in the 401 WQC or non 404 permit?
