Dear Mr. Fairman:
On August 22, 2023, Division of Water Resources (DWR) Winston-Salem Regional Office (WSRO)
staff Jesse Barnes conducted a routine compliance evaluation inspection at the Countryside NC
MHP WWTP in reference to NPDES permit NC0056201. ORC Paul Smith was present during the
inspection. A review of the subject permit, records, treatment components, receiving waters, and
plant operations and maintenance was completed. This review reflected compliance with the
subject permit.
Items of Concern
1. NPDES permit NCO056201 became effective February 1, 2019 and will expire at midnight
on January 31, 2024. At the time of this inspection, a new permit renewal application had
not yet been received from the permittee.
2. The facility has been upgraded since the last permit issuance, with approval from DWR.
The system components listed on the supplement to permit cover sheet are no longer
accurate. Please ensure the system components are listed and described accurately in the
upcoming permit renewal application.
3. Recent monitoring reports have listed the certified laboratory used as "NC00030", which is
no longer valid. Please list the most up-to-date names and NC lab certification numbers for
all field and non -field labs used to report permit -required parameters.

Mr. Smith stated that the septic tank sludge depths are regularly measured with a sludge
judge and facility owners are notified once sludge depths exceed 3 feet, but at times the
septic tanks have not been pumped frequently enough. Recent permit violations of BOD
and fecal coliform limits may be attributed to excessive sludge depths in the septic tanks,
which could be ameliorated with more frequent pumping. Some surface grease and solids
were observed in the septic tanks during the inspection.
Please be aware that the Division may take enforcement action against any permittee who fails

assessed against any person who violates or fails to act in accordance with the terms, conditions,

staff Jesse Barnes conducted a routine compliance evaluation inspection at the Countryside NC MHP
WWTP in reference to NPDES permit NC0056201. ORC Paul Smith was present during the
inspection. A review of the subject permit, records, treatment components, receiving waters, and plant
operations and maintenance was completed. This review reflected compliance with the subject permit.

4. Mr. Smith stated that the septic tank sludge depths are regularly measured with a sludge judge

be attributed to excessive sludge depths in the septic tanks, which could be ameliorated with more
frequent pumping. Some surface grease and solids were observed in the septic tanks during the
inspection.
Other observations:
There are no special conditions for the subject permit. Access to the plant site is restricted to the
general public by a locked metal chain -link fence. The inspector was granted access to all areas for
inspection.
All required records are stored off site and maintained by AQWA. All requested records were
available to review electronically via tablet and appeared to be complete.
Field parameters of pH, temperature, and dissolved oxygen are performed by Mr. Smith under NC
certified field lab # 5169 — Smith Industries. Bench and calibration data records for field parameters
performed during July 2023 were reviewed and appeared to be complete. Non -field parameters are
performed by Pace Analytical LLC, NC certified lab # 633. Non -field parameter data are stored off site
at AQWA offices and were available for review electronically via tablet during the inspection.
Laboratory data sheets and chains -of -custody for July 2023 were reviewed and appeared to be
complete.
The system components listed on the permit are no longer accurate, as the old activated sludge
package plant has been demolished and completely removed. The new Orenco AdvanTex AX-Max
Treatment System is now in place and operational. The current system consists of two 10,000 gallon
septic tanks that provide primary treatment, one 10,000 gallon equalization tank, two Orenco
AdvanTex AX-Max treatment units with textile biological media that can be operated in parallel or in
series, and a Sanitron UV disinfection system, along with all pumps, valves, piping, controls, and
other appurtenances to complete the treatment system.

The equalization tank and its pumps appeared to be in good condition and operation properly during
the inspection. Float controls and alarms were not tested duringthe inspection.
The facility utilizes two Orenco AdvanTex AX-Max treatment units for biological treatment. After leaving
the septic tanks and EQ tank, wastewater is passed through textile biological media in the AX-Max
units and recirculated through the units multiple times before flowing to the UV disinfection system.
The AX-Max treatment units appeared to be operating properly during the inspection.
The facility disinfects with a Sanitron UV light system consisting of two bulbs operating in a series with
independent quartz sleeves, housings, and manual scrubbers for each bulb. The UV disinfection
system appeared to be operating properly during the inspection. Sufficient backup bulbs were
available on site during the inspection.
Flow is monitored and reported with an in -line analog effluent flow meter. The flow meter is replaced
annually with a newly -calibrated meter. Records of new flow meter calibration and installation were
not available during the inspection, but Mr. Smith stated that records are maintained at AQWA offices
and available upon request.
The facility has an automatic propane generator that is automatically tested every week and can
power the entire treatment system. The primary power source is not interrupted during automatic
tests of the generator.
Effluent samples are grabbed from a spigot located after the UV disinfection system. Effluent and
in -stream sampling appeared to have been conducted in compliance with the subject permit.
