Describe the attachments or comments: Revisions for the R-5014 Project. Site 2 - Additional Riprap on the triple crossline resulted in approximately 33 sq.ft. of additional open water impacts. The additional riprap was extended to the ROW. See attached drawing of Sheet 4 of 41. Site 14 - A stormwater pipe near Site 14 had an extension installed on the inlet side, as shown in the permit drawings. The current plans had the pipe remain in place and no work on the outlet end of the pipe. However, the elevation of the new roadway grade at this pipe has resulted in a very steep slope. The Resident's Office has requested authorization to install a 4' extension on this pipe and regrade the shoulder to a 3:1 slope. This would include riprap stabilization on the slope. The extension and shoulder widening would result in approximately 40 sq. ft. of perm. open water impact from fill and approx. 40 sq. ft. of temp. impact from dewatering. See attached drawing of Sheet 25 of 41. Site 15 - The contractor included a riprap dissipator pad at the outlet of a stormwater drain near Site 15 across from Billy's Seafood. This resulted in approximately 100 sq. ft. of unauthorized impacts in Coastal Marsh. This impact is near the Collington Creek Mitigation site; however, the mitigation site was not impacted. There was previously some riprap at the stormwater drain, but it was not documented, and the exact area is unknown. See attached drawing of Sheet 25 of 41.
