The report shall also provide a listing and summary of all enforcement actions taken or pending during the quarter. The quarters begin on January 1 st, April 1 st, July 1 st and October 1 st . The report shall be submitted within 30 days after the end of each quarter. Submit additional sheets if necessary or if multiple treatment receiving facilities are involved.
