NORTH CARouNA Direcror Environmental Quality August 16, 2023 George Crew II — Senior Pastor Lattisville Grove Baptist Church, Inc 1701 Jimmy Ed Road Hurdle Mills, NC 27541 SUBJECT: Compliance Inspection Report Lattisville Grove Baptist Church WWTF Non-discharge Permit No. WQ0022785 Orange County Dear Permittee: The North Carolina Division of Water Resources conducted an inspection of the Lattisville Grove Baptist Church WWTF on June 19, 2023. This inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in Non-discharge Permit No. WQ0004353. The findings and comments noted during this inspection are provided in the enclosed copy of the inspection report entitled "Compliance Inspection Report". Division records indicate a treatment system consisting of a 1,200-gallon equalization tank with a 40 gpm pump and audio/visual alarms, 3,000-gallon septic tank; a 684-square foot (ft2) subsurface sand filter, a tablet chlorinator, four 18,000-gallon storage /pump tank; audio and visual high-water alarms; a moisture sensor, flow meter, a 1.04-acre spray irrigation area (nine heads), and all associated piping, valves, controls and appurtenances. During the inspection, the following was observed: Schedule (Section I of permit): 1. The permit expires on October 31, 2023. According to the permit Section L 3. a renewal was to be submitted six months prior to the expiration of the permit, by April 30, 2023. Performance (Section II): 2. No setback issues were observed. 3. The irrigation field was sufficiently fenced. Operation and Maintenance (O&M) (Section III): 4. During the inspection, a rain/ moisture sensor was observed. 5. The system appeared to be operational at the time. The ORC was not on-site to assist in assessing the system. 6. The chlorinator had the correct tablets and was in good condition. 7. Chad Leinbach services the system weekly. No records were provided or reviewed for maintenance. Please provide maintenance records and when the tank was last pumped. 9. The July NDAR report indicated the irrigation meter was malfunctioning. Please provide documentation that the meter has been calibrated annually and the meter has been repaired. 10. The monthly reports are submitted to NCDEQ as required. 11. The annual soil results were provided to NCDEQ. The results recommended the irrigation field is limed. Please maintain the irrigation field as the soil report recommends. Inspections (Section V): 12. Chad Leinbach performs maintenance on the system weekly and as needed. General Conditions (Section VI): 13. The facility continues to be owned by the Lattice Grove Baptist Church and annual fees have been paid. No violations have been reported in the past five years. This report is a notification of the results of the compliance inspection. Within 30-days receipt of this letter, submit a written response to this office addressing the issues noted in items 8, and 9 above. Please be aware that non-compliance with your permit could result in enforcement action by the Division of Water Resources for these and any other violations of State law.
