This office needs to be notified if and when this is accomplished. ICA issued for sediment loss off site onto downstream property and juridicitional areas at several locations due to erosion control measures not being installed adequately after site was cleared and grubbed. Loss locations noted during this inspection: Sta. 916+00 Rt-L Sta. 924+00 Rt-L Sta. 930+00 Rt-L Sta. 931 +00 Rt-L Remarks and Recommendations: Met on site with Gerald (Summit) and Dennis (Summit) for on site erosion control site review due to sediment loss at sta. 916-+00 Rt-L reported by the CEI Resident's office. Installation of erosion control devices was not complete in this area and recent rainfall event produced sediment loss downstream to wetland off R/W. 7 0 8/14/2015 11:33 AM Page 1 of 3 Sediment loss was in excess of 5 gallon bucket therefore requires NC DWR be notified within 24 hours. **DWR notified 8/14. -Recommend all of the project be inspected throughly per NPDES requirements to insure no other sediment losses occured during recent rainfall. -During discussions, the timing of EC installation after clearing and grubbing was questioned. Section 225-2 in the Standard Specifications for Roads and Structures states that EC devices are to be installed before any land disturbing activity. In most cases, this requires clearing to get to the locations for installing the EC devices and staging the installation as work progresses. In any event, the areas should not be grubbed, removing all cover and root mats, without concurrent installation of the perimeter EC devices. Additional devices per the EC plans can be installed as work progresses to direct storm water runoff to basins, such as temp diversions and slope drains. Sta. 916+00 Rt-L: -Install a large Type A Rock Sediment Dam (str 634.01 in NCDOT Roadway Standard Drawings) across clean water diversion. -Install Type B Rock Checks in clean water diversions per the approved plan. -Stabilize clean water channel across the roadway section with adequate ground cover. -Sediment loss onto downstream property wetlands is greater than a five gallon bucket however do not recommend removing sediment at this time. Wait for the ponded area to dry and assess accumulation. If it remains under water, the site can be pumped down. This will be discussed in the next review. Sta. 924+OORt-L, Sta. 930+OORt-L, and Sta. 931+OORt-L: -Install erosion control devices on site per the approved plan. -Complete the installation of skimmer basins, diversion ditches, silt fence, etc. -Stabilize clean water diversion through project with adequate ground cover. -Install Type B Rock checks or wattles in clean water diversion per the approved plan. -Recommend installation of the large Type A Sediment Dam across the outlet area to help trap sediment. Sta. 941 +00 Richardson Creek: -Complete installation of skimmer basins, diversion ditches, rock checks, wattles, etc., per plan and as needed to prevent impacts to the creek. -Install outlets in silt fence per NCDOT specifications. (Currently stone dumped on top of silt fence.) -Stabilize all bare areas with adequate ground cover. Permit Site 62 -Silt fence has been installed below skimmer basin with wattle outlet. -Silt fence has been installed along creek at end of culvert out toward roadway. -Re-install 57 stone outlet with either hardware cloth or rip-rap behind 57 stone to insure it will not be washed into the creek. -Recommend digging small Type B Basin in front of 57 stone outlet to help with settling out sediment and slowing runoff down prior to 57 stone outlet. -Stockpiled material should be consolidated and tracked in to minimize erosion. Also may need to temporarily mulch stockpiles. Permit Site 56 -Delineate permitted areas with orange fencing. -Install erosion control devices per approved plan. -Crews currently driving through permitted areas with equipment as it was not delineated. Skimmer basin 3B-192 -Install temporary diversion ditches to basin per the approved plan. (Pipe under driveway is covered up or has been removed.) Overall Comments: -Crews have been working and are continuing to work on corrective actions from August 4, 2015 site inspection. The contractor had the disturbed areas seeded and mulched prior to the rain. This probably helped in reducing the overall sediment losses but will not substitute for the required EC devices. Make sure all corrective actions have been noted in NPDES weekly reports with date of completion. -John (MBC) to advise through email when all corrective actions have been completed from previous inspection. -Maintain all erosion control devices per weekly NPDES reports. -Maintain all erosion control devices on site as needed. (Clean debris off rock checks, rip-rap filter stone ring 8/14/2015 11:33 AM Page 2 of 3 pipe inlet protection, etc.) -Continue to install/maintain construction entrances at all entrances/exits from site. -Make sure all permitted areas are delineated to insure that crews will not drive equipment throught permitted areas or work without proper erosion control devices in place while work in or around these areas. PCN issued on 8/4 needs to be reviewed and comments provided to us concerning the delineation of the permitted sites and measure to prevent impacts until work begins. Per discussions with RK&K on the EC design: The clean water channels designed across the roadway area will not function as clean channels if they are grubbed and equipment drives thru them. These areas should be shaped up, seeded and mulched and rock silt checks or wattles installed across them every 40' or so to slow the flow. Equipment crossing shuiold be limited to one area in the upper section behind as many of the EC devices as possible. Installing large rock sediment dams at the outlet ends will help trap sediment. These dams can be installed during the grubbing phase prior to other EC devices to help prevent sediment loss until the other measures are installed. This project will be reinspected by Aug 20 to determine if sufficient progres has been made. Additional ICA Extensions can be issued to allow for more time as needed. 8/14/2015 11:33 AM Page 3 of 3
