The Division has determined that your application is incomplete and cannot be processed. The application is on -hold until all of the following information is received: 1. Please provide an agent authorization form signed by Frederick Slagle granting you signature authority. A sample agent authorization form, which may be used, is attached. [15A NCAC 02H .0502(a)(11)] 2. All disturbance below the normal pool lake level must be conducted such that the lake is isolated from the disturbed area. Please confirm the work will be conducted while the lake is drawn down or describe the measures used to meet this condition. [15A NCAC 02H .0506(b)(2)] Please clarify the total area of impacts below the normal pool lake level. The application form states 300 square feet, but the diagram provided with the application indicates 480 square feet (3 feet x 160 feet). [15A NCAC 02H .0502] Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information for the proper consideration of the application. Please provide your response by October 11, 2023. If all of the requested information is not received, the Division will be unable to approve the application and it will be denied as incomplete. The denial of this project will necessitate reapplication to the Division for approval, including a complete application package and appropriate fee.
