Dear Mr. Hollingshead:
On September 7, 2023, Brian Lambe from the Wilmington Regional Office of the Division of Energy, Mineral, and
Land Resources(DEMLR),conducted a compliance evaluation inspection SRM Ready Mix Concrete Jacksonville
Plant 49. The site drains to Sandy Run Branch,which is currently classified as Class SC, NSW Waters in the White
Oak River Basin.
The following observations were noted during the DEMLR inspection (please see the attached addendum for
additional information about your permit):
1)Stormwater Pollution Prevention Plan(SPPP)
A Stormwater Pollution Prevention Plan (SPPP) has been developed, recorded,and properly implemented.
The copy of the SPPP onsite was not updated as required by permit.
2)Qualitative Monitoring
Qualitative monitoring has been conducted and recorded in accordance with permit requirements.
3)Analytical Monitoring
Analytical monitoring has not been conducted and recorded in accordance with permit requirements.
2022 produced no flow results for two sampling periods.2022 should have had three total sampling periods
due to new permit and change to quarterly sampling. During inspection,the pond,outfall 2 was overgrown
and could not locate outfall.
Request reinspection. Please contact this office immediately to schedule an inspection with Brian Asberry and
Quinn Vaughan onsite.

NPDES Stormwater General Permit may be subject to a civil penalty assessment of up to$25,000.00 per day
