TOBY VINSON, Jr. NORTH CAROLINA Interim Director fnvtrontnenta[Quality September 19, 2023 5&W Ready Mix Concrete Company, LLC Attn Hank Gay PO Box 872 Clinton NC 28329-0872 Subject: Compliance Evaluation Inspection NPDES General Stormwater Permit NCG140027 Jacksonville East Thompson Street Facility Onslow County Dear Mr.Gay: On September 7, 2023, Brian Lambe from the Wilmington Regional Office of the Division of Energy, Mineral,and Land Resources(DEMUR),conducted a compliance evaluation inspection Warsaw Oil Processing Facility located in 3464 US Hwy 117 S, Pender County, North Carolina. The site drains to Sandy Run Branch,which is currently classified as Class SC, NSW Waters in the White Oak River Basin. The following observations were noted during the DEMUR inspection (please see the attached addendum for additional information about your permit): 1)Stormwater Pollution Prevention Plan(SPPP) A Stormwater Pollution Prevention Plan (SPPP) has been developed, recorded, and properly implemented. Outfall 2 needs additional measures to treat wastewater. Illicit discharge behind the product storage area needs to be pushed to outfall 2 or submitted as an additional outfall. 2) Qualitative Monitoring Qualitative monitoring has been conducted and recorded in accordance with permit requirements. 3)Analytical Monitoring Analytical monitoring has not been conducted and recorded in accordance with permit requirements. No sampling events have been recorded via eDMR. Mr Gay reports he has registered but has not uploaded data. Outfall 2 needs data analysis.Submit records to Wilmington regional office immediately. Please be advised that violations of the NPDES Stormwater General Permit may be subject to a civil penalty assessment of up to$25,000.00 per day for each violation. Need to review sampling results. Additional measures may be needed. The BMP consists of a 2500 sq ft basin with two rock check dams.Very concerned about the pH of the discharge. Rock washdown area has created a discharge point that needs to be captured and pushed to outfall 2.
