I was looking over the informa?on you provided on 8/7/23. Everything looks good except for a few small issues:
Please correct the following:
- Part D1 of the Applica?on – Please provide the contact informa?on (email address and phone number) for the
applicant signing official, Meridith Gordan.
- Part D2 of the Applica?on – Please provide the registered agent informa?on.
- Part F7 of the Applica?on – Since the applicant, Brookside Chris?an Academy, is a corpora?on, please provide
the requested items demonstra?ng that the applicant is in good standing with the NC Secretary of State and
confirming the name and posi?on of the signing official.
Please provide the following:
- 2x hard copies and 1x electronic copy of the revised pages of the Applica?on.
- 2x hard copies and 1x electronic copy of the Secretary of State informa?on
- 2x hard copies and 1x electronic copy of any other items needed to address these comments.

Hand Delivery:
Please reach out to me prior to hand delivering a submission to make sure that I (or someone else in my group)
will be able to receive the submission. Do not leave the package in the foyer with the security guard.
NOTE: Hard copies should not be sent to a Regional Office. Doing so will delay the review process and the
submission package may be lost while being sent from the Regional Office to me in the Central Office.

Since these are really minor things, once I get the hard copies, anything else that may have changed from addressing
these issues, and the documents are uploaded to Laserfiche (Provided everything is s?ll in order), we can issue the

permit without doing another formal review. The quickness of the next review for this project is dependent on reviewer
availability.

We received your post-construc?on stormwater \[Authoriza?on to Construct (ATC)\] permit applica?on for the subject
project on 8/7/23. Reviews under the Fast-Track permi?ng process can take up to 30 calendar days to complete. You
can see where this project stands in the review queue by emailing Jim Farkas (He has an autoreply set up that has the
review queue on it). Please upload all documents that you mailed into our office through the link provided below. Be
sure to choose the Central Office as the Primary Contact office.

IMPORTANT NOTE: Please ensure that each file is uploaded under its appropriate file type. Addi?onal file types can be
entered into the electronic form by clicking on the “Add Another Post-Construc?on File Upload” link directly below the
“State Stormwater Informa?on Uploads” sec?on of the form. YOU MUST CLICK ON THE “ADD ANOTHER POST-
CONSTRUCTION FILE UPLOAD” TO ADD ADDITIONAL FILE TYPES TO THE SUBMISSION. Merely changing the file type
dropdown as you upload the various files (without adding addi?onal file uploads) will upload all of the files as the last
file type selected, the submission will get flagged during review, and you will need to resubmit the files. Please do not
upload en?re plan sets through this form, but rather upload the relevant individual sheets under the appropriate file
type. Selec?ng the correct file type ensures that the files are properly named in the electronic repository and it helps
expedite the review process. NOTE: We do not require and we discourage the upload/submission of extraneous plan
sheets (such as ligh?ng plans, u?lity plans, striping plans, sight distance plans, etc…) as these files are not needed for
review.

Provided that the applicant’s signature is an original, wet ink on the mailed hard copies, this looks good.
Also, if the rest of the plans remain the same, you can just mail in that revised page of the plan set so that I may add it to
the exis?ng set of plans we have.

We received your application for a new Fast-Track post-construction stormwater permit on 7/20/23. At this time, the
submission is not administratively complete and cannot be accepted for review in its current condition. Please provide
the following submittal items within 30 calendar days (by close of business on 8/24/23) so that we may accept the
project and begin the review process, otherwise, the project will be returned as administratively incomplete.
2x hard copies and 1x electronic copy of the Application with the following corrections made (These items are
required per 15A NCAC 02H .1043(3)(a) and per Section F of the Application):
Section E, 3 - Please list the correct river basin that this project is located in (This project appears to be
located within the Cape Fear River Basin). This is required per 15A NCAC 02H .1043(3). REVISED
Section G – Please provide the applicant’s/financially responsible party’s original, notarized and sealed
signature in this section of the Application. NOTE: One original signature was received, but two were
needed.
2x hard copies and 1x electronic copy of a construc?on sequence that discusses how any future development on
the project may be phased. This is required per 15A NCAC 02H .1043(3)(g) and per Sec?on F, 5 of the
Applica?on. The construc?on sequence does not need to be lengthy, just a brief descrip?on of how future
development on the project (beyond what is shown on the plans) will be phased. If no future development is
planned for the project, you can simply state that in the construc?on sequence. This item is different than the
E&SC sequence of construc?on. REVISED-ATTACHED
2x original hard copies and 1x electronic copy of a site plan depicting the boundary of the project or project
phase currently being permitted, including the locations of SCMS, streams, wetlands, and buffers. This item is
required per 15A NCAC 02H .1043(3)(f) and per Section F,4 of the Application. NOTE: Surface waters, veg.
setbacks, and wetland delineation (or a note on the plans stating that there are no wetland located on-site”
needs to be shown on the plans. NOTE ADDED TO C4.0
NOTE: This is an administrative completeness review, not a detailed engineering review, there may be other issues that
may be brought up during the engineering review.
If these items are received within the stipulated timeframe, we can accept the project and begin the review process.
Once accepted, reviews under the Fast-Track permitting process can take up to 30 calendar days per statute.
