The application for the subject project listed the applicant as Green Meadows, LLC. After issuing the approval letter for
the subject project to Green Meadows, LLC, we found on the NC Secretary of State website that the company known as
Green Meadows, LLC has been administratively dissolved. We also noted that the structural fill and mining permits were
issued to Green Meadow, LLC, which is a registered company with the Secretary of State. Please clarify the name of the
applicant so that we may issue a corrected water quality certification approval letter.
