This email serves to notify you of the loss of sediment into the below noted permitted sites on I-5110, R-2413A and R-
2413B. These loss were a result of a large rain event that came through Guilford County last night which averaged 3”+
of rainfall (The site sustained between 2.3”, at Bryan Blvd., and 5.8”, at Deboe Road -Y7-, of rain.

I-5110:
-Site #1: Significant Loss (Reviewed by Jerry Parker in the field on 8-7-15 and retrieval limits have been

determined. Clean-up in process starting 8-7-15)
R-2313A:
-Site #2: No loss into jurisdictional stream (Basin failure resulting in loss near the stream but not into the stream,

Jerry Parker reviewed this area on 8-7-15 and provided limits for retrieval. Clean-up in process starting 8-7-15)
--Y1- Sta. 108+50 Rt at Drainage Structure 2702 (Loss near stream that is not permitted on this project, but

appears to be jurisdictional, no loss in stream, , Jerry Parker reviewed this area and provided limits for
retrieval. Clean-up scheduled to begin 8-8-15)
-Site #10: Significant Loss (Reviewed by Jerry Parker in the field on 8-7-15, Jerry stated that he would like for

Dave Wanucha with NCDENR to come out to evaluate clean-up limits and methods. Tentatively scheduled for
Monday, 8-10-15)
-Site #16: No apparent loss into jurisdictional stream (Basin failure resulting in loss near the stream. To be

evaluated by NCDOT and agencies next week.)
R-2413B:
-Site #4: No loss into jurisdictional stream (Basin failure resulting in loss near the stream in the floodplain. To be

evaluated by NCDOT and agencies next week.)

Please let me know if you have any questions or concerns. We are currently in the process of repairing/replacing
measure that were overwhelmed and securing the remainder of the site. The cleanup and safe up operations will
continue through the weekend with all roadway forces and supplemental bridge crew forces.
