This assessment is based upon the following facts: a review has been conducted of the Discharge Monitoring Report (DMR) submitted by Brookdale Senior Living DBA Brookdale Hendersonville East for the month of April 2023. This review has shown the subject facility to be in violation of the discharge limitations and/or monitoring requirements found in NPDES WW Permit No. NC0069370.
