Bay River Metropolitan Sewerage

We are submitting the following sewer service extension application for Bay River MSD, 2023
Blackwell Point Road Sewer Extension. The purpose of this project is to supply a sewer service to 288
Blackwell Point Road. This project consists of the following:
A. Approximately 1,325 LF of 2-inch PVC force main.
B. 1 Low Pressure S.T.E.P. System.
We are submitting the following in this application, and Cover Letter (Item B):
(Item C) McDavid Associates, Inc. check #041436, dated August 25, 2023 in the amount
of $480, payable to NC Department of Environmental Quality for the application
fee.
(Items A&D) Two (2) copies of the completed Alternative Sewer Extension (Form:
ASEA-0416) Application.
There are no wetlands or surface waters within 50 feet of the sewer system
extension, wetland boundaries are shown on each page of the plans (Exhibit 1).
4. (Item F) Flow Tracking/Acceptance Form (Form FTSE 10-18) from Bay River
Metropolitan Sewerage District (Exhibit 2).
(Item G) Two (2) sets of plans "2023 Blackwell Point Road Sewer Extension, Contract
No. 1 Sanitary Sewer," dated June 19, 2023, consisting of 10 pages, (MAI Plan
File B-1910 GOLD).

6. (Item H) Two (2) sets of contract specifications for "2023 Blackwell Point Road Sewer
Extension, Contract No. 1 Sanitary Sewer," dated June 19, 2023.
One compact disc containing plans and specifications.
8. (Item I) Engineering Calculations. The calculations consist of the following:
(1) Kentucky Pipe calculations for Low Pressure S.T.E.P. system (Exhibit 3).
(2) Pump Cycle times and pump run times for LPSS Simplex S.T.E.P. System
(Exhibit 4).
(3) Force main velocities based on pipe type and flow and pump operating
probabilities (Exhibit 5).
(4) Flotation calculations for the LPSS Simplex S.T.E.P. Septic Tank (Exhibit
6).
9. (Item J) Downstream Sewer Evaluation (Exhibit 7).
10. (Item K) Two (2) color copies of a USGS Topographic Map and street level map showing
the project area (Exhibit 8).
11. (Item M) The septic tank/pump well has a storage volume of approximately 398 gallons
above the pump on level, which exceeds the average daily flow of 360 gallons per
day, therefore it exceeds 24 hours of storage (Exhibit 9).
12. (Item N) A final environmental document is not applicable.
