On November 2, 2007, the Wilmington Regional Office received a request for a plan revision of
Stormwater Management Permit Number SW8 .:990306. The revisions include the following:
Plan Revision
CONSTRUCTION OF 15,745 SQUARE FEET OF BUILT-UPON AREA ASSOCIATED WITH A
NEW ACCESS ROAD TO GATEWAY BANK 5,911 SQUARE FEET OF THIS ACCESS ROAD
WILL DRAIN TO POND 1 AND 9,834 SQUARE FEET OF THIS ACCESS ROAD WILL DRAIN
TO POND 3. THIS PLAN REVISION WILL REDUCE THE FUTURE AMOUNT OF BUILT-UPON
AREA FOR POND 1 FROM 167,499 SQUARE FEET TO 161,588 SQUARE FEET.AND rr WILL
REDUCE THE FUTURE AMOUNT OF BUILT-UPON AREA FOR POND 3 FROM 246,872
SQUARE FEET TO 237,038 SQUARE FEET. HOWEVER,THE OVERALL BUILT-UPON AREA
FOR THIS DEVELOPMENT WILL REMAIN THE SAME. THERE WILL BE NO NET INCREASE
IN BUILT-UPON AREA.

This office has determined that a formal permit modification is not required for the
proposed changes. We are forwarding you an approved copy of the revised
plans for your files. Please add the attached plans to the previously approved
plan sheets.
Please be aware that all terms and conditions of the permit issued on April 10,
2001, remain in full force and effect. Please also understand that the approval of
this revision to the approved plans for the subject State Stormwater Permit is
done on a case-by-case basis. Any other changes to this project must be
approved through this Office prior to construction.

Dear Ms. Lewis:
We are enclosing two (2) sets of plans for your review reflecting impervio
area of the Gateway Bank temporary acces road at Research Park. The project was
previously permitted under SW8 990306 on e be4 6, with a total impervious
area for Pond #3 of 285,841 SF and a total of 205,644 SF for Pond #1. On November
14, 2006, your office issued a plan revision for the entrance road infrastructure to
Gateway Bank resulting in 13,535 SF of impervious area to Pond 3. An off-site permit
for Gateway Bank was issued on November 14, 2006 under off-site permit No. SW8
060944 permitting 25,434 SF of impervious area to Pond #3. The new access road to
Gateway Bank will result in 9,834 SF to Pond #3, therefore, leaving a future impervious
area balance in Pond#3 of 237,038 SF.
On April 7, 2005, an off site permit (SW8 050321) was written for the FNB Building in
the Pond #1 drainage area for 38,145 SF. The new access road will result in 5,911 SF
of impervious area to Pond #1, therefore, leaving a future impervious area balance of
161,890 SF.

and diligence was used in the observation of the project construction such that the construction was observed to be
built within substantial compliance and intent of the approved plans and specifications.
The checklist of items on page 2 of this form is included in the Certification.
Noted deviations from approved plans and specification:
Pond#4: As per original permit, Stormwater Pond #4 was an expansion of an existing pond.
Therefore, the vegetation within the pond and on the northeast side slope was allowed
in the permit.

Certification Requirements:
11 1. The drainage area to the system contains approximately the permitted acreage.
The drainage area to the system contains no more than the permitted amount of built-upon area.
3. All the built-upon area associated with the project is graded such that the runoff drains to the system.
