Marion City Engineer Intern Division of Water Resources State of North Carolina Department of Environmental Quality Division of Water Resources 15A NCAC 02T .0400 — SYSTEM -WIDE WASTEWATER COLLECTION SYSTEMS INSTRUCTIONS FOR FORM CSA 04-16 & SUPPORTING DOCUMENTATION Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant Division Policies. Failure to submit all required items will necessitate additional processing and review time. For more information, visit the System -wide Collection System Permitting website: General — When submitting an application to the Municipal Permitting Unit, please use the following instructions as a checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided boxes will help produce a quicker review time and reduce the amount of requested additional information. The Applicant shall submit one orisinal and one copy of the application and supporting documentation. The copy may be submitted in digital format. A. Cover Letter g Submit a cover letter listing all items and attachments included in the permit application package B. No Application Fee Required ➢ No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit ➢ The appropriate annual fee for systemwide wastewater collection system permits may be found at: ➢ Annual Non -Discharge Fees C. System -Wide Wastewater Collection System (FORM: CSA 04-16) Application: ® Submit the completed and appropriately executed System -wide Wastewater Collection System (FORM: CSA 04- 16) application. Any unauthorized content changes to this form shall result in the application package being returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long as the attachments are numbered to correspond to the section and item to which they refer. ❑ If the Applicant Type in Section I.3 is a Privately -Owned Public Utility, provide the Certificate of Public Convenience and Necessity (CPCN) from the North Carolina Utilities Commission demonstrating the Applicant is authorized to hold the utility franchise for the area to be served by the wastewater collection system, or ❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an application for a franchise has been received and that the service area is contiguous to an existing franchised area or that franchise approval is expected. ❑ If the Applicant Type in Section I.3 is a corporation or company, provide documentation if it is registered for business with the North Carolina Secretary of State. D. General Information: ➢ The Authorized signing official listed in Section I.4 should match with that of the Applicant certification page in accordance with 15A NCAC 02T .0106(b). There are a small number of spare parts and equipment that are maintained at the maintenance shop such as: • Replacement Impellers • Mechanical seals • Cutter rings and star blades • Packing • Floats • Portable Generators • Various electrical control devices • Some spare pumps • Level transducers if available CITY OF KING SEWER PUMP STATION CONTINGENCY PLAN The City of King has 8 duplex pump stations, 2 triplex pump stations and 1 quadplex pump station. The City would respond to each pump failure at each pump station in a similar way. Each pump station has a primary and a secondary pump. Failure of the primary, secondary, or both pumps shall initiate this contingency plan with the goal of replacing the failing pump as soon as possible, unless the station has triplex or quadplex pumps. If both pumps fail to activate, alarms will be activated. The alarms will contact the water plant. The ORC will then be called to investigate to determine the reason for the backup or overflow. The City of King can do some minor pump repairs on site; however, it is in the best interest of the City to contract most pump repairs. The City will contract with a local contractor in the event of pump failure in order to replace or repair the pump as soon as possible. The City will keep specific details for each pump on site in to expedite pump replacement. If a pump failure occurs, the ORC will check first for any sanitary sewer overflow. If an overflow has or is occurring, the ORC will begin containment procedures for the SSO. Containment is to be in compliance with the Sanitary Sewer Overflow procedures contained within the Collection System Program. If an S S O is not or has not occurred, then the ORC will begin diagnosis of the pump failure. If the source is not obvious or is beyond the City's capabilities to repair, then the ORC will contact a local contractor to expedite the repair. We have a 24/7 emergency number that is to our Water Treatment Plant. The number is 336-924-8363. For all emergencies after call is made to the water plant, they then dispatch on call personnel and the ORC's will be contacted of the emergency and the location. 2. Once the call is made, personnel are dispatched, and they respond as soon as they are contacted. 3. Sewer equipment list and supplies: A. Sewer jet trailer/washer and sewer camera B. Repair supplies are kept at the city maintenance shop. C. Portable generators for smaller stations, larger stations are equipped with generators. D. Pumps are available for smaller stations. E. Cutters and ceramic seals are available on hand for pumps. F. Pressure washers, sewer jet and floats are on hand. 4. The wastewater maintenance department in the city budget has allocated funds for unexpected emergencies. 5. Site sanitation: hydrated lime is used to treat overflow and chlorine tabs are used in the event wastewater reaches surface waters. 6. Spill overflows are reported immediately to NCDENR, then if the overflow is over a certain amount and reaches surface waters it is reported to the proper newspapers and television station if this becomes necessary. 7. The City has a bypass pump around system for two of our stations which are located at our Spainhour Road station and our Goff Road station. For the other stations without a bypass pump around system we call in pump trucks to handle the situation.
