Box 189 Smithfield, NC 27528 -0189 Subject: REQUEST FOR ADDITIONAL INFORMATION Brookside Subdivision Dear Mr. Evans: Donald R. van der Vaart Secretary DWR #15 -0674 Johnston County On July 9, 2015, the Division of Water Resources (Division) received your application dated June 26, 2015 requesting a 401 Water Quality Certification from the Division for your project. The Division has determined that your application is incomplete and cannot be processed. The application is on -hold until the following information is received: 1. The site plan needs to clearly show and label Zones 1 and 2 of the Neuse Riparian Buffer as well as the jurisdictional wetlands. A key indicating the differentiation of these two resources should be provided on the site plan. Clearly showing these features will likely require full -size plan sheets. [15A NCAC 02H .0502(b)] 2. Provide a cross - section detail of the culvert showing the provision for aquatic life passage. [15A NCAC 02H .0506(b)(2)] 3. Section E. Stormwater Management and Diffuse Flow Plan of the application is only partially filled out and is incorrectly filled out. The project is adjacent to Neuse Riparian Buffers, and therefore, diffuse flow must be achieved. In addition, since the percent imperviousness of this project is 30 %, a stormwater management plan is a requirement of the 401 Water Quality Certification, and it is a requirement of Johnston County. If all of the requested information is not received in writing within 30 calendar days of receipt of this letter, the Division will be unable to approve the application and it will be returned. The return of this project will necessitate reapplication to the Division for approval, including a complete application package and the appropriate fee. Please respond in writing within 30 calendar days of receipt of this letter by sending four copies of all of the above requested information to the 401 & Buffer Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699 -1617. Please be aware that you have no authorization under the Water Quality Certification Rules for this activity.
