Minimum Design Criteria for Gravity Sewers (latest version), and the Minimum Design Criteria for the Fast -Track Permitting
of Pump Stations and Force Mains (latest version). Although other professionals may have developed certain portions of this
submittal package, inclusion of these materials under my signature and seal signifies that i have reviewed this material and
have judged it to be consistent with the proposed design.
NOTE — in accordance with General Statutes 143-215.6A and 143-215.613, any person who knowingly makes any false
statement, representation, or certification in any application package shall be guilty of a Class 2 misdemeanor, which may
include a fine not to exceed $10,000, as well as civil penalties up to $25,000 per violation. Misrepresentation of the application

To whom it may concern,
BGE inc., on behalf of Forestar (USA) Real Estate Group Inc., submit this Fast Track Sewer
System Extension Application for the Brightwood Farm — Phase 5A project. The project is
located at 6816 Preakness Parkway, Whitsett, Guilford County, NC 23229 and consists of 140,
4-bedroom, townhome units and associated infrastructure. There is 3,125 LF of 8" SDR sewer
proposed to serve the community and tie into the existing sewer system within the Brightwood
Farm development. Included in this submittal are application for acceptance from the City of
Burlington and the City of Greensboro. This is because the sewer lines are owned by the City of
Burlington, but the treatment is by the City of Greensboro.
