Description of projects area and activity: This permit verification authorizes 197 linear feet (0.039 ac) of permanent stream impacts to Jonathan Creek associated with maintance to existing bank stabilization. 29.59 Linear feet (0.017 acre) of temporary stream impacts for the installation of coffer dams and construction access, and 167.70 linear feet (0.097 ac) of temporary stream impact for dewatering. Instream work shall not be concudcted during Trout Moritorium (October 15 — April 15). SUBSURFACE INVESTIGATION AND GEOTECHNICAL REPORT: THE CONTRACTOR SHALL FAMILIARIZE HIMSELF AS TO THE EXISTING SITE CONDITIONS PRIOR TO BIDDING THE PROJECT, NO SUBSURFACE INVESTIGATION OR GEOTECHNICAL REPORT IS PROVIDED. 8, OSHA: CONTRACTOR SHALL COMPLY WITH CURRENT OSHA REGULATIONS. 9. INSTALLATION: THE CONTRACTOR SHALL PROVIDE TEMPORARY ACCESSTOTHE CONSTRUCTION SITE DURING INSTALLATION. ALL WORK ASSOCIATED WITH PROVIDING ACCESS SHALL ALSO INCLUDE ANY ASSOCIATED ELECTRICAL COSTS, FENCE INSTALLATION, NECESSARY FILL MATERIAL, DRIVEWAY CULVERTS, NECESSARY SILT FENCING, AND ALL FINAL GRADING AND SEEDING ON THE SITE UNLESS OTHERWISE NOTED. 10. TEMPORARY UTILITIES: CONTRACTOR SHALL BE RESPONSIBLE FOR OBTAINING ALL TEMPORARY UTILITIES NECESSARY TO PERFORM THE CONSTRUCTION. NOTES: • THE CONTRACTOR SHALL TEMPORARILY MOVE UNDERGROUND UTILITIES WHEN THE UTILITY CAN BE MOVED OUT OF THE WAY OF ACTIVE CONSTRUCTION ACTIVITY WITHOUT DAMAGING THE UTILITY. WHERE THIS CANNOT BE ACCOMPLISHED, THE CONTRACTOR SHALL NOTIFY THE TOWN ENGINEER IMMEDIATELY AND SHALL WORK WITH THE OWNER AND THE AFFECTED UTILITY COMPANY TO DEVELOP A PLAN OF ACTION. WHERE ADDITIONAL WORK IS REQUIRED BY THE CONTRACTOR, THE CONTRACTOR SHALL SUBMIT A CHANGE ORDER FOR APPROVAL BY THE OWNER PRIOR TO COMMENCING CORRECTIVE ACTIONS. • THIS DOES NOT RELIEVE THE CONTRACTOR FROM LIABILITY RELATED TO DAMAGED UTILITIES OR INTERRUPTION OF SERVICE AS WRITTEN IN BELOW, AND IN THE CONTRACT DOCUMENTS. 11, COMPACTION OF TRENCHES: ALL TRENCHES SHALL BE COMPACTED TO A MINIMUM OF 95% OF MAXIMUM DENSITY IN TOWN AND NCDOT RIGHTS OF WAY TO WITHIN 12' OF FINISHED GRADE. DENSITY TESTS SHALL BE PAID FOR BY THE CONTRACTOR. ALL TRENCHES FOR ROAD CUTS WILL BE COMPACTED BY MECHANICAL MEANS, 12. SEEDING AND MULCHING: PAYMENT FOR SEEDING AND MULCHING THE AREAS DISTURBED WRING CONSTRUCTION SHALL BE INCLUDED IN THE PRICE ESTABLISHED IN THE BID. 13. DRIVEWAY REPLACEMENT: THE CONTRACTOR WILL BE REQUIRED TO REPLACE ANY EXISTING GRAVEL, CONCRETE, ASPHALT OR PAVERS THAT HAVE TO BE REMOVED AND REPAIR THESE SECTIONS TO ORIGINAL OR BETTER CONDITION. 14. SUBMITTALS: THE CONTRACTOR SHALL OBTAIN WRITTEN APPROVAL FOR ALL SHOP DRAWINGS AND SUBMITTALS FROM THE ENGINEER PRIOR TO BEGINNING ANY INSTALLATION WORK. 15. SHOPDRAWINGS: SHOP DRAWINGS SHALLBE SUBMITTED FOR ALL PRECAST STRUCTURES, METAL CASTINGS INCLUDING HATCHES, MANHOLE RINGS AND COVERS, VALVE BOXES, AND CATCH BASIN FRAMES AND GRATES; ROLLED EROSION CONTROL PRODUCTS, AND GEOTEXTILES. MANUFACTURERS CERTIFICATIONS SHALL BE SUBMITTED TO THE ENGINEER CERTIFYING THATALL PUMPS AND PIPE MEETS THE PROJECT SPECIFICATIONS. CONCRETE MIX DESIGNS, THE DETAIL SPECIFICATIONS AND BITUMINOUS CONCRETE PLANT MIX DESIGNS OF THE DETAIL SPECIFICATIONS SHALL BE SUBMITTED TO THE ENGINEER FOR APPROVAL. NONE OF THE ABOVE LISTED MATERIALS SHALL BE USED ON THE PROJECTS UNLESS THE ENGINEER HAS GIVEN THE NECESSARY APPROVAL FOR THAT PARTICULAR MATERIAL. THE CONTRACTOR SHALL SUBMIT ALL SHOP DRAWINGS,CERTIFICATIONS, AND MIX DESIGNS REQUIRED WITHIN THIS CONTRACT. 16, AS -BUILT DRAWINGS AND SPECIFICATIONS AT THE JOB SITE: THE CONTRACTOR SHALL MAINTAIN, IN READABLE CONDITION AT THE JOB SITE, ONE COMPLETE SET OF WORKING DRAWINGS AND SPECIFICATION FOR THE WORK, INCLUDING ALL SHOP DRAWINGS. SUCH DRAWINGS AND SPECIFICATIONS SHALL BE AVAILABLE FOR USE BY THE OWNER OR ITS REPRESENTATIVE ATALL TIMES. THIS SET SHALL BE MARKED, OR NCTES ACCEPTABLE TO THE ENGINEER PROVIDED, IN ORDER TO REFLECT AS -BUILT CONDITIONS. THE CHANGES INDICATING SUCH CONDITIONS SHALL BE KEPT CURRENTAT ALL TIMES.
