The Division of Energy, Mineral, and Land Resources (DEMLR), received a complete, Stormwater Management Permit
Authorization to Construct (ATC) Application for the subject project on August 23, 2023. Staff review of the plans and
specifications has determined that the project, as proposed, will comply with the Stormwater Regulations set forth in
Title 15A NCAC 2H.1000 - 1062 rules. We are forwarding ATC Permit No. SWA000214, dated August 24, 2023, for the
construction of the subject project and the stormwater system. This permit shall be effective from the date of issuance
and shall be subject to the conditions and limitations as specified therein, and does not supersede any other agency
permits that may be required.

The original signed permit package and plans are being mailed to the permittee, Ralph Dickson, at the following address

We received your post-construction stormwater Authorization to Construct (ATC) permit application for the subject
project on 6/16/2023. Reviews under the Fast-Track permitting process can take up to 30 calendar days to complete.
You can see where this project stands in the review queue by emailing Jim Farkas (He has an autoreply set up that has
the review queue on it). Please upload all documents that you mailed into our office through the link provided
below. Be sure to choose the Central Office as the Primary Contact office.

IMPORTANT NOTE: Please ensure that each file is uploaded under its appropriate file type. Additional file types can be
entered into the electronic form by clicking on the “Add Another Post-Construction File Upload” link directly below the
“State Stormwater Information Uploads” section of the form. YOU MUST CLICK ON THE “ADD ANOTHER POST-
CONSTRUCTION FILE UPLOAD” TO ADD ADDITIONAL FILE TYPES TO THE SUBMISSION. Merely changing the file type
dropdown as you upload the various files (without adding additional file uploads) will upload all of the files as the last
file type selected, the submission will get flagged during review, and you will need to resubmit the files. Please do not
upload entire plan sets through this form, but rather upload the relevant individual sheets under the appropriate file
type. Selecting the correct file type ensures that the files are properly named in the electronic repository and it helps
expedite the review process. NOTE: We do not require and we discourage the upload/submission of extraneous plan
sheets (such as lighting plans, utility plans, striping plans, sight distance plans, etc…) as these files are not needed for
review.

Hi Kaitlin,

We looked back at our UPS documents (see attached) and they say that everything was delivered to the "mail room"
back on 06/16. I also re-checked the shipping address and everything looks correct. Can you please double check and
see if it may have gotten misplaced?

 Section A: In order to qualify for Fast-Track your project must bear the signature and seal of a professional
licensed pursuant to either Chapter 89A or 89C of the NC General Statutes. Without this, your project is
not eligible. If your project does in fact have this signature and seal with these requirements, please the
check the box on the application.
 Section H: The signature for your design professional must be an original signature.
 All of the other items are good to send over as they appear in the attachments.

 Section B, 3 – Please provide accurate information concerning the physical location (street
address) of the project. This is required per 15A NCAC 02H .1043(3)(a)(ii). NOTE: This comment is
in reference to Section B, 3. Please provide a more specific street address.
 Section H – Please provide the design professional’s original, notarized and sealed signature in
this section of the Application.
 2x original, signed hard copies and 1x electronic copy of a site plan depicting the boundary of the project
or project phase currently being permitted, including the locations of SCMS, streams, wetlands, and
buffers. This item is required per 15A NCAC 02H .1043(3)(f) and per Section F,4 of the Application. NOTE:
Please revise include wetland delineation or a note on the plans stating that there are no wetlands
located on-site. NOTE: Please revise to include a signature on these site plans.
 2x hard copies and 1x electronic copy of a construction sequence that discusses how any future
development on the project may be phased. This is required per 15A NCAC 02H .1043(3)(g) and per
Section F, 5 of the Application. The construction sequence does not need to be lengthy, just a brief
description of how future development on the project (beyond what is shown on the plans) will be
phased. If no future development is planned for the project, you can simply state that in the construction
sequence. This item is different than the E&SC sequence of construction.
 2x hard copies and 1x electronic copy of documentation from the NC Secretary of state, or other official
documentation, which supports the titles and positions held by the persons listed in Section c, 2 per 15A
NCAC 2H .1043(3)(b). The corporation or LLC must be listed as an active corporation in good standing
with the NC Department of the Secretary of state; otherwise, the application will be returned.

NOTE: This is an administrative completeness review, not a detailed engineering review, there may be other
issues that may be brought up during the engineering review.

If these items are received within the stipulated timeframe, we can accept the project and begin the review
process. Once accepted, reviews under the Fast-Track permitting process can take up to 30 calendar days per
statute.
