On July 11,2023,the North Carolina Division of Water Resources(DWR)received an after-the-fact
Notification of Intent to Construct or Operate Injection Wells(NOI) for one air sparge well that was
installed April 24-25,2023 and began operating May 1-5,2023 at the above referenced facility. Based
upon communications concerning this matter,the lack of submitting the NOI prior to commencement of
injection activities was an oversight.
DEFICIENCY—FAILURE TO SUBMIT NOI PRIOR TO INSTALLING/OPERATING AN UIC WELL
Title 15A NCAC 02C .0225(e)requires that a NOI be submitted to DWR two weeks prior to installing
and operating groundwater remediation injection wells deemed `Permitted by Rule'. A NOI was not
received by DWR two weeks prior to injection activities.
REQUIRED CORRECTIVE ACTION FOR THIS DEFICIENCY
Per 15A NCAC 02C .0225(e), a NOI was received by DWR on July 11,2023. This satisfies the
requirement per the rule and no further action is required regarding the NOI at this time. A permit
tracking number of W10400602 was assigned for this well for future reporting of injection related data.
