This inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in NPDES WW Permit No. NC0066796. A summary of the findings and comments noted during the inspection are provided in the enclosed copy of the inspection report. The Compliance Evaluation inspection was conducted by Division of Water Resources staff from the Asheville Regional Office. The following deficiency(s) was noted during the inspection: Inspection Area Description of Deficiency Effluent Pipe Effluent pipe could not be found at the time of the inspection. Remedial actions should have already been taken to correct this problem and prevent further occurrences in the future. The Division of Water Resources may pursue enforcement action for this and any additional violations of State law. To prevent further action, carefully review these deficiencies and address the causes of non-compliance to prevent the recurrence of similar situations. NCO066796 I11 12I 23/07/31 117 18 ici cJ Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) Rachel Rose and Lauren Armeni, with the Asheville Regional Office (ARO), met with Robert Barr (ORC) to conduct a Compliance Evaluation Inspection on July 31, 2023 for the Leicester Elementary School WWTP. This inspection was conducted to determine whether the facility is being operated and maintained in compliance with NPDES Permit No. NC0066796. The following item was noted during the inspection: Sand Filter: At the time of the inspection, the sand filter was full of gravel which covered the majority of the piping; however, the ends of the distribution lines were elevated above the gravel (picutres attached). There was no flow from the distribution lines at the time of the inspection. There was some vegetation on top of the gravel, but not enough to hinder the functionality. Effluent Pipe: The effluent pipe could not be found due to excessive vegetation alnog the bank of the creek. The following deficiency was noted at the time of the inspection: Effluent Pipe: At the time of the inspection, the effluent pipe was not able to be evaluated because it could not be found. There was high overgrowth of vegetation making it too difficult for the ORC and the inspectors to find the effluent pipe.
