On March 24, 2015, the Division of Water Resources (Division) received your, application dated March 23,
2015, requesting a 401 Water Quality Certification from the Division for the subject project. The Division
has determined that your application is incomplete and cannot be processed. Please note, two previous
attempts to send this correspondence via certified mail to the address provided in your application were
returned to this Office. The application is on -hold until all of the following information is received:
1. Stormwater Requirements
Per Water Quality General Certification 3890, this project is required to provide a stormwater
management plan to protect water quality. Please see Appendix A of the General Certification,
available at: http://Dorta1.ncdenr.org/web/wq/swp/ws/401/certsandoermits/ cs , and submit a
stormwater management .plan that is in accordance with these requirements. [15A NCAC 02H
.0506(b)(5) and (c)(5)].
2. Plan Details
Please provide cross section of wetland impacts sites including fill slopes. [15A NCAC 02H .0502(8)]
Pursuant to Title 15A NCAC 02H .0506 (b)(5), (c)(5), and (g), the applicant shall furnish all of the above
requested information for the proper consideration of the application. If all of the requested information is
not received in writing within 30 calendar days of receipt of this letter, the Division will be unable to
approve the application and it will be returned. The return of this project will necessitate reapplication to
the Division for approval, including a complete application package and the appropriate fee. Please
respond in writing within 30 calendar days of receipt of this letter by sending three copies of all of the
